Grade 3 ptosis is severe sagging in which the nipple lies well below the inframammary fold and lies at the bottom of the breast, at the lowest hanging part of the parenchyma (cells in the ducts and glands that produce milk) along the inferior contour of the breast. Pseudoptosis (glandular) occurs when the breast tissue falls below but areola is in a good position.
Pseudoptosis is typically caused from the weight of the eyelid skin pushing down the eyelid margin so that it covers part of the pupil. Eyelid ptosis on the other hand is when there is weakness of the elevator muscle that elevates the eyelid and this creates a large eyelid fold as well.
I prefer an incision technique on the skin in order better access the muscle that opens the eyelid directly (anterior approach). The eyelid crease can be recreated more precisely this way. The posterior approach is done from the undersurface of the eyelid but in my opinion is not as precise.

It's hard to judge by this picture as you are "smizing" or smiling with your eyes.Based on the picture, I doesn't look like you would benefit from upper eyelid filler. You make benefit from ptosis surgery without blepharoplasty if the lid position is low when you aren't smiling. Less is more ...
Ptosis surgery is complex and so is its reversal but it can be done with some limitations. Discuss with your surgeon and see an expert oculoplastic surgeon.
