Grade 3 ptosis is severe sagging in which the nipple lies well below the inframammary fold and lies at the bottom of the breast, at the lowest hanging part of the parenchyma (cells in the ducts and glands that produce milk) along the inferior contour of the breast. Pseudoptosis (glandular) occurs when the breast tissue falls below but areola is in a good position.
Pseudoptosis is typically caused from the weight of the eyelid skin pushing down the eyelid margin so that it covers part of the pupil. Eyelid ptosis on the other hand is when there is weakness of the elevator muscle that elevates the eyelid and this creates a large eyelid fold as well.
