Hi, Thank you for your inquiry. What I find with side pictures is that oftentimes patients are looking for subtle changes mostly to the height of their nose or a more defined tip which is not as easy to see from the frontal view.

The chin can be reduced in height by removing a wafer of bone and plating the remaining bones together with microplates. A side view will help determine whether the bone should be moved slightly forward before plating it. I usually do it from inside the mouth but it can also be done from under the chin.

The exotic upward slant to the eye area is accomplished by a combination of lateral canthopexy, in which the outside corner of the eyes are attached to a higher position along the eye socket. To prevent crowding in this outer area, a temple lift is performed at the same time in order to create the outer area "sweep". 

If you are unable to contact the original surgeon, my best guess is that a permanent suture was used to anchor the eyelid corner to the orbital bone. The suture can be removed under local anesthesia in most cases. The skin in this area is very thin so it's not unusual for a permanent suture to show through. 

Undereye filler should not interfere with upper eyelid ptosis surgery. If your surgeon offers this procedure, you could also consider micro fat grafting the lower lid area if you are already undergoing surgery.

The left eye muscle is not opening as well as the right side - this is corrected by reinserting the muscle more tightly to the lid cartilage. Your brows will descend as a result of this surgery, since your brows will not need to pull so much to help the eyes see better. The drop will occur over both eyes. Hence, the excess skin should be done on both eyes, in anticipation of the brow descent. If you like the current position of the brows, then a brow suspension will need to be performed to prevent the brow descent.

As long as there isn't any active infection or abscess at the teeth roots, I would proceed with the surgery. If you have any loose teeth, let the anesthesiologist know - it would be better if everything were stabilized prior to surgery.

It appears to be displaced lower lid fat that was transposed into the tear trough groove. The easiest fix for this is to go back in and remove a little bit of the bulging fat. I would wait at least to the three month mark, possibly 6mo before doing this. Probably best would be to have your original surgeon do this since he knows what was done. The next surgeon should get a copy of the operative report before doing a revision, if you choose to go to another surgeon. 

Yes, you look like a great candidate for the two procedures. The chin implant will help stretch out the skin so that it will be tighter, and the neck lipo will further improve the neck contour. Your chin only needs anterior projection (not vertical change) so that puts you into the category of patients that do well with chin implantation.
