Since you had a favorable result with temporary fillers, you may wish to consider permanent fillers which could replicate that nasal appearance, but without the necessity for repeating the procedure. You would benefit from a rhinoplasty. This will take care of the bulbous tip and the dorsal deformity of your nose. The surgery takes 2 hours to perform under general anesthesia.

Thank you for your question. Unfortunately, you got duped by less than professionals. You are an excellent candidate for rhinoplasty. I think it is one of the most beautifying surgeries that we do as plastic surgeons. A large portion of my practice is dedicated to rhinoplasty surgery. It is important to only see plastic surgeons with extensive experience in rhinoplasty, ie perform at least 6 per month and have performed hundreds of cases. Also, critically evaluate on-line galleries to see if their art form resonates with yours. Rhinoplasty is definitely a grey haired plastic surgeon's sport. You should be able to have an excellent result. Best wishes to you.

I would consider dermabrasion which will thin out the skin and scar as an initial treatment. It may need to be done a few times for best result

One option that has not been tried is injection with kenalog (a steroid) and/or 5 fluorouracil (a different anti-scar medication). Another option which is frequently used to improve scars is treatment with laser. Sometimes these are used in combination (topical steroid massaged into the skin just after the laser).

I see no reason why you can’t have rhinoplasty surgery.
This will probably require an open rhinoplasty with Cartlidge grafting. If the problem bothers you enough to be willing to have that type of surgery, then consult with plastic surgeons who have extensive rhinoplasty experience.
Did you have a thread lift? If so that may make things more complicated and add uncertainty.
Schedule in person consultations with local plastic surgeons, who have at least 1 to 2 decades of rhinoplasty experience. Look for providers to focus exclusively on rhinoplasty surgery, or whose practice has a heavy emphasis on rhinoplasty procedures.
During each consultation, ask each provider to open up their portfolio and show you their entire collection of before and after pictures of previous patients, who had similar nasal characteristics to your own period
An experienced provider should have no difficulty showing you the before, and after pictures of at least 50 previous patients.
Being shown a handful of pre-selected images, representing only the best results of a providers career is insufficient to get a clear understanding of what average results look like in the hands of each provider, what your results are likely to look like or how many of these procedures they’ve actually done.
Tip rhinoplasty surgery is a finesse procedure that is complex and should not be performed by those who don’t have extensive experience if you want to minimize the chance of undesirable outcomes.
