I had an SMAS facelift 8 weeks ago. I still have slight jowling and deep lines from corners of mouth. Can I get a revision?

In a slim person, in particular with moderate to significant sun damage it is more difficult to eradicate the marionette lines and jowls, because the necessary tension applied to tissues can produce either a "pulled" appearance, increased visibility of the scars around the ears or even displacement of the ears. After 6-9 months from the original surgery it is possible to contemplate a revision, and some surgeons feel strongly that deep plane techniques may offer some answers in a situation like yours. Alternatively, chemical peels can tighten the facial skin and produce improvement along the lines you describe, possibly in combination with injectable fillers. Together, they can also produce a smoother contour of the jawline.  Sculptra treatment is another option that can help for a situation like yours.

Thank you for your question.  I am sorry to hear that you are disappointed after your recent operation.  My advice is to wait at least 5 to 6 months before considering any revision surgery. A facelift is an invasive procedure; therefore the body needs time to recover and you will have to allow time for the result to settle. At that stage, it will be appropriate to discuss your options with your surgeon and politely voice your concerns.  Without an accurate physical examination or at least some photos to understand better the situation, I am afraid that it would neither be possible nor appropriate to give you any more advice. All the best.Andrea Marando

So 8 weeks is very soon after such an extensive operation. The tissues are firm and lumpy and the scar are stiff. Most surgeons wouldn't even think about revising an operation until 6 months after surgery. One more thing to consider is your age. At 70, no matter how hard you pull on skin and tissues to tighten them, they will only be tightened so far. Without meeting you I can't tell if this is your issue but a face lift in a 70 year old is less perfect than a facelift in a 50 year old. Give healing a chance and ask your surgeon about your concerns.
