Good Afternoon doc, i am 24 yrs old woman. i want to tell you that there is a deep valley like hole between my lower eyelid and my cheekbone. i've always had eyebags but it used to be no valley, but whenever i look at the mirror recently the valley is visible, it makes me look older and look tired. im so stressed about it because "im not looking good anymore". why is that happening doc? what should i do? is there anyway to get rid of it without filler because i can't afford it.

Hello and thank you for your question.  You are a great candidate for under eye fillers.  In your case, you seem to have loss of fat in the under eye area, causing a deep, sunken in, look.  With fillers such as Juvederm or Restylane, that loss of volume can be replaced to give a smooth rested look.  I suggest seeing an experienced injector who specializes in facial aesthetics for best results.

Dark under eye circles can make you look tired and older than you are. I would use a combination protocol that includes topical Melarase creams first, followed by a combination of injectable fillers, fractional laser, and RF to blend out the eyelid hollowness and tighten the skin.

Under eye hollows can be permanently corrected with surgery or temporarily camouflaged with fillers (Restylane, Belotero, Volbella) which can also help nasolabial folds.There are great non-surgical options to fill in under eye depressions and hollowness. Using dermal fillers, a liquid eye-lift can be performed. A dermal filler such as Restylane would be placed in the cheek and tear trough to hide the depression or bag underneath your eye. 
Surgery would require a fat-repositioning lower lid blepharoplasty. In this surgical procedure, fat from the eye area is moved into the tear trough depression filling the actual depression.

A combination approach will give the most improvement. Fillers, lasers, and microneedling/PRP can improve under the eyes. My patients try to get non surgical treatments to avoid surgery. Thank you for your question and images. You do have a slightly unique orbital anatomy but you are not alone. You have prominent bony orbital rims and lovely full cheeks. Although in some people this can give the appearance of hollowness around the eyeball. Given your age and that I can not see any herniation of the lower eyelid fat pads, you essentially need volumisation. Injectable fillers would be a good non surgical option but you may also consider fat grafting around the eyelids. My advice is to find someone you trust and try the orbital fillers first. In time if you like the results you may consider investigating more permanent options such as fat grafting. Good luck.RegardsDr Guy Watts

Hello and thank you for your question.  Many factors determine whether we develop bags and under eye discoloration. As we get older, it’s not uncommon to see the changes start developing as early as our mid to late 20s. Minimally invasive treatments such as tear trough fillers and a variety of lasers can significantly improve the appearance of what you are describing.  Although 25 is quite young to consider a lower blepharoplasty, having an inpatient consultation with a board-certified facial plastic surgeon may be worth considering. Together, you can determine which treatment is best to address your concerns and help you achieve your aesthetic goals.

Filler would be a great option for you to help even out the area under the eyes.  The treatment is done in the office and the results last up to one year.  Eye creams may help improve tone and texture around the eye, but they will not be effective at adding the needed volume.  See an experienced injector in your area to learn more.

First of all you have very good cheekbones and projection with a lovely shaped face. For genetic reasons you have a relative lack of soft tissue (fat and muscle) in your under eye area. This is sometimes called the tear trough or under eye hollows or under eye dark circles, depending on how it looks to you. I really like your description of under eye valleys! Surgery is not required. You can have filler treatment in the under eye area using a hyaluronate gel. It is not permanent but once you have completed the treatment, and this may take two or three small out patient sessions, then it usually lasts over a year and just requires occasional top ups.An oculoplastic trained cosmetic eyelid surgeon with experience in under eye tear troughs (valleys) treatment.
