Jowls like those pictured can be helped with filler injections, cheek and neck lift surgery (even a mini lift can be very effective), and even pre-jowl chin augmentation or laser resurfacing when combined with a lift procedure. Kybella is not safely injected into the jowls due to the risk of nerve injury. A combination approach will give the most improvement. I recommend fillers for the jowls themselves but also lifting around the mouth and cheeks to build underlying anatomical support. In our practice we combine sculptra with other fillers "layered" on top and lasers, rf treatments, J Plasma and peels to get the most conservative natural look. Please see a facial sculpting expert. Thank you for your question and photo. Kybella is the world’s first FDA approved PERMANENT fat destruction injectable. I like to call it surgery in a syringe. Off label, Kybella can be used by advanced injectors to destroy fat that is forming jowls. I am unable to fully assess your needs because you only have one photo which cuts off your midface as well as your lower face contour. It may be that filler injection in your midface will lift your lower face. It may also be that Kybella in the jowls will melt the fat and contour your jaw. Hard to tell… I recommend you start with a consultation to discuss your needs with an experienced injector. Further more, because your skin scaring will benefit from a resurfacing procedure like CO2 laser resurfacing or Vivace Microneedling Radiofrequency, I should mention that these modalities will also tighten your skin and resolve the sagging in the lower face.Best Regards, Dr. Bonakdar

Kybella (Deoxcholic Acid) is the first non-surgical, FDA approved injection to reduce submental fat. Distracting fat in the jowls (considered off-label use) relieves pressure on the lower face resulting in more youthful jawline. Kybella should always to perform by a skilled injector as to not damage the Marginal Mandibular Nerve, which may lead to nerve paralysis.Some of the side-effects include temporary bruising, swelling, redness and numbness.
Kybella is an excellent treatment for jowls as long as there is enough fat in the area. The best results are usually from a combination of filler, PDO threads, and Kybella. Filler in the jawline will provide some lift and definition, and PDO threads are a great way to lift the jawline as well as marionette lines, and nasolabial folds.
Kybella is FDA approved for submental fat (double chin) but it is also used for the fat ion the sides of the jaw, sometimes referred to as jowls. If there is sufficient fat in that area Kybella can help.
