I had my surgery earlier this year, I am still under the 1 year make and acknowledge they aren't fully done. But I can see a clear separation between my natural tissue and the implant. I have silicone implants under the muscle. What is the likelihood this will settle? At it's current state I am not satisfied with this result and would like to make a change to fix it.

Your surgeon who has measured you will be the best one to tell you if a higher profile is needed for the larger breast. It is best that the implant sits well in the pocket. Going larger often means using a higher profile implant with a more narrow base to fit the base of your breast.
If you look back at your original paperwork, you probably signed something that you read and agreed with about payment for revision surgery. Yes, it is fair to ask you to pay for the surgery center and anesthesia because this money does not go to your surgeon, and it is very kind of your surgeon...
I would say that overall the incision designs (especially the one into the armpit) are not acceptable. It is difficult to fully determine what is going on without performing a physical exam, but there is certainly some unnatural aesthetics (with all due respect). You would likely need a revision...
