Based on your photos, your dorsal hump involves both your nasal bone and your nasal cartilage. Once that is removed, that will automatically lead to a wider nasal appearance on the operating table. To narrow your nose after hump removal, osteotomies would need to be performed. Operations like this site typically performed under general anesthetic, not local anesthesia. Thank you for your photographs. It is possible to lower your nasal bump under twilight anesthesia, plus numbing the entire area with local anesthesia. However, consideration should be given to what is the safest way to proceed and what would give you the best experience. As the nostrils are connected directly with the airway, I would always recommend an anesthesiologist be present for airway control. Certainly the safest and most expedient way to have the surgery would be to have the anesthesiologist give you generally anesthesia, which allows them to fully control your airway and allows you the best experience. With twilight anesthesia there may be periods of time where your airway would need to be suctioned, disrupting the flow ‘of ‘the procedure and making your experience less than ideal. The cost of a rhinoplasty depends upon the surgeon and the location you’re having it done. Have a detailed discussion with your plastic surgeon about these issues. Best wishes.
I wish it were simply shaving down the bridge that would work. You really need a full rhinoplasty including breaking your nasal bones to get a decent result. Otherwise it’s not even worth doing
You must undergo a full rhinoplasty procedure when shaving down the dorsal hump due to the fact that once the Hump has been shaved down, you will left with a flat top, open roof nasal deformity. Osteotomies placed in the nasal bones are required to close that open roof. You will also need your tip to be balanced with the new bridge line. You can certainly have a reduction rhinoplasty if you would like.The procedure can be done using local anesthesia, local anesthesia with sedation or general anesthesia.Each surgeon will have their own preference for how to do the procedure.Plasty surgery is fairly complex and patience.
