Based on your photos, your dorsal hump involves both your nasal bone and your nasal cartilage. Once that is removed, that will automatically lead to a wider nasal appearance on the operating table. Thank you for your photographs. It is possible to lower your nasal bump under twilight anesthesia, plus numbing the entire area with local anesthesia. However, consideration should be given to what is the safest way to proceed and what would give you the best experience. As the nostrils are connected directly with the airway, I would always recommend an anesthesiologist be present for airway control. Certainly the safest and most expedient way to have the surgery would be to have the anesthesiologist give you generally anesthesia, which allows them to fully control your airway and allows you the best experience. With twilight anesthesia there may be periods of time where your airway would need to be suctioned, disrupting the flow ‘of ‘the procedure and making your experience less than ideal.
