I think my nose is to skinny and pointy on it's bridge, I think it's wide on the sides, and fat at the bottom near by pointy nostrils. I think I should describe the upturned look with a flat nose bridge, what do you think my end result should be that I will describe to my doctor?

Depending on your condition, Accutane may help with some swelling in your nasal skin, especially if it is thick. If you still require a steroid injection after commencing Accutane, discuss the feasibility of this with your dermatologist. I hope this helps. Sincerely, Dr. Joseph
Based on your photo, you appear to have nostril asymmetry on front view. Your left nostril is higher than your right, and there are some small tip asymmetries as well. You are seeing this likely because you have thin nasal skin. You do not appear to have an overly wide nasal bridge appearance....
Many people feel much better after having their external nasal valve repaired with cadaver rib cartilage. If you're not breathing well through your nose, this is a discussion you should have with your surgeon or another reputable rhinoplasty specialist. Wishing you well moving forward....
