Thank you for your question. In my opinion, the best outcome for your breasts would be a lollipop mastopexy to lift and reshape the breast, as well as reposition the areola to a new position. An in person consultation with a board certified plastic surgeon would be needed to determine what is best for you.
Based on your pictures it looks like you can benefit from a full breast lift, an in person consultation would be best for a better exam and evaluation.
Dear samjake,My preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. Thank you for you question and photos, The Bellesoma Method is a current technique to lift and/or reduce breasts without using implants or the ugly vertical scars. The incisions are hidden around the areola and in the inframammary crease. The vertical incision has been completely eliminated. The areola incision heals almost invisibly due to the fact that there is hardly any tension on the closure. With this particular technique, ducts are preserved along with nerve supply maintaining nipple sensation and the ability to breast feed. Because the breast is reshaped internally and is secured to your chest muscles, upper pole fullness is possible without an implant. Attached is a patient who underwent The Bellesoma Method. Please note that her tissue was used to create upper pole fullness and cleavage without any foreign materials (mesh, implants, etc). You are a great candidate for a breast lift. A vertical or lollipop incision should work out well, though a small horizontal incision in the breast crease sometimes helps with the final shape. The donut incision will never give you the shape you want and I rarely use it. Discuss all with your plastic surgeon. Good luck.

Your own breast tissue can be used to create upper pole fullness. In regards to your wish pic I think it is important to realize that woman has not had a breast lift. Your breast would require incisions that would leave scars down the center of your breast. These scars would fade over a year but would not disappear completely. Be sure to see a board-certified plastic surgeon to get the best plan and treatment.I hope this helps :-)

With thatbeing said, based on your photographs, the best shape and contour could beachieved with a vertical or wise pattern breast lift. I rarelyrecommend a Bennelli or circumareaolar (donut) lift. Over time, theareola tends to enlarge with a thickened scar. The other even worseproblem with a circumareaolar lift is that it flattens out the shapeof the anterior breast over time. A vertical or wise pattern lift can result in a very nice shape if good technique isused. The size of the implant is based on your desiredbreast size/shape, your chest wall measurements, and soft tissuequality. This decision should be based on adetailed discussion with equal input from both you and yoursurgeon. Make sure you specifically look at before andafter pictures of real patients who have had this surgery performed by yoursurgeon and evaluate their results. The most important aspect is tofind a surgeon you are comfortable with. Hello and thank you for the question. Ingeneral, incision type depends on the degree of ptosis (sagginess). Ptosis isgraded by the vertical position of nipple in relation to inframammary crease.There are a lot of factors involved in the decision making process, however,incision pattern could range from a periareolar type (donut) for mild ptosis,to vertical (lollipop) for moderate ptosis and Wise-pattern(vertical incision in the midline and horizontal incision at the crease)for server ptosis. Based on the degree of your ptosis, I would recommend a Wise-pattern incision. Your surgeon should discuss all these and various surgicaltechniques in details and explain to you the pros and cons of each procedure.Ultimately, the final operative plan should be based on a detailed discussionand a common understanding between you and your surgeon. You should becomfortable with your surgeon's skills and be able to trust him/her. Please askyour surgeon to show you before and after pictures of real patients that had asimilar procedure. Based on your pictures looks like can benefit from a scar revision, you can have this done under local at the office, consult with your PS.
I usually do not put tape over the incisions keep the area dry and no ointments, but every surgeon has different post op care instructions follow up with your PS to discuss your concerns.
