I can clearly see the septal cartilage bone, the skin does not cover it properly, its leaned to one side and this makes my nose look weird. One side is more bulbous than the other. It looks like my nose is leaning to one side. What can i do about it, to make it orderly(regular)+question: I think that my nose seems big, because its stick out from my face too much, or because the tip is bulbous. Can you please give me your opinion about it, as it seems like I'm the only one who thinks that, I'm confused.
Answer: Rhinoplasty What you are pointing out is what is referred as a hanging columella. The columella is the part between the nostrils. The lower end of the septal cartilage is pushing the columella down and is sticking out of the right nostril. Very simple to fix, but it is surprising how many surgeons either ignore it or try to approach it with complicated solutions such as cartilage grafts. As part of an overall rhinoplasty the hanging columella is corrected by removing the excess cartilage at the lower end of the septum as well as nasal lining.

This can be made straighter and you nose refined so it looks smaller, but natural at the same time. Be sure to see a rhinoplasty specialist for consultation.

Answer: Deviated septum The nasal septum is deviated towards right and needs to be fixed. You have a bulbous tip and need tip plasty. Rest of the nose is good. Tip-plasty Tip-plasty is a great procedure to address a bulbous tip only and leave the rest of the nose alone. It can be accomplished through an open or closed approach. The goal is to refine the tip and make it smaller and smoother. In tips with strong cartilages but thin skin, all that is needed is removing some of the cartilage and softening the rest by gently crushing it (morselizing). In tips with thick skin and lot of soft tissue or fat, the excess fat and soft tissue is removed along with removing some of the cartilage. Click on the video link below. Regards Dr. J
