Can filler in the tear trough help with a herniated fat pad? (Photo)

It is my opinion that fillers are best used to treat hollowed tear troughs and loss of volume in the area.  With herniated fat pads, fillers may camouflage the problem but a lower blepharoplasty will address it directly.  There are different approaches to treatment of the fat pad including shrinking or excising the fat or relocating the fat to fill in the hollows.  I recommend you have a formal consultation with a facial plastic surgeon or oculoplastic surgeon who is experienced in lower blepharoplasty.

A filler in the tear trough can help some patients while it may not help some at all. It really depends on the cause of the problem, size of the fat pad, and amount of loose skin. In your case, I believe there’s a good opportunity for improvement as there’s good looking skin that appears tight. It depends on your exam so see an expert. Good luck! Dr. Aldo

Filler can help to camouflage herniated fat pads but only to a point. If the day pad is too large, it can sometimes be more noticeable after filler. This is determined on your exam and it’s hard to tell from just a photo. However, your fat pad doesn’t look that big so it’s possible filler could help.

Thanks for the question and for the photo.  We always recommend that you have a consultation with a board certified dermatologist or plastic surgeon to first have a thorough exam and second to learn your treatment options. If you have a herniated fat pad, having filler injected may mask that for a short period but if it needs correction, then perhaps that is the best route to go and then see if you are in need of fillers.  See the right provider and good luck.

Fillers can help soften them bit, but if the fat is herniated, then treat the herniation. Camouflaging may not be fully accomplished with just fillers.

Thanks for your question. Whether or not there is room for improvement is fairly easy to examine. When you find there is an 'empty' space which you might feel on top of the bone of the orbital rim, mostly at 6 o'clock and 4 o'clock looking from the (left) pupil, you can expect that filler in this particular area will lessen the relative height of the herniated fat pad. Furthermore, the skin can be strengthened (Palma technique) to reduce folds and transparency.
