Can saline or sterile water injections help dissolve Restyalne Defyne in upper eyes instead of Hyalulronidase?

I thoroughly agree that hyaluronidase, particularly of the human derived kind (Hylenex), is the most appropriate way to dissolve unwanted hyaluronic acid-based injectable fillers and volumizers. I have been using hyaluronidase for years and have found it to be a safe and effective product for this purpose. Although generally felt not to affect normal surrounding hyaluronic acid, I have actually seen it do so. If there is a known allergy to this product or if there is concern about the above, as appears to be the case here, subcision (to physically break up the filler masses) combined with saline injections are a safe alternative approach, although gratifying results via this method typically require two or three treatment sessions. Very dilute concentrations of intralesional triamcinolone are sometimes additionally needed, but these must be used with extreme caution to avoid steroid atrophy. Make sure that you consult with a board certified aesthetic physician with experience and expertise in this field. Best of luck. 

Hyaluronidase is an enzyme that breaks down Hyaluronic Acid. Water or Saline do not have the same effect on fillers. Hyaluronidase is safe and effective if injected properly and used in conservative amounts. Rather than relying on horror stories on RealSelf, I would strongly suggest that you seek in-person consultation with an experienced injector, preferably a Face and Neck Plastics Surgeon. Best, Dr. Anil Shah

Have this stuff removed with hyalronidase. No it does not wreck your skin. Yes you do need to actually go to someone who knows what they are doing. The Restylane Defyne is not like regular Restylane. It is much more runny and will require more enzyme. Most of the so-called enzyme horror stories are from folks who were under treated by well meaning doctors. There is no substitute to going to someone who really knows what they are doing.

Saline will have no effect on the Defyne. I would do hyaluronidase. It is simple and has no side effects despite what you see on Realself. There is no scientific evidence to show it has any long term negative effects. In patients with occlusion, thousands of units are injected and they do fine afterwards.

So saline will be ineffective at removing hyaluronic acid-based filler. Most of these fillers last between six months and 24 months at the most so I don’t think you will have to wait 4 years. Having said that, don’t be too nervous of hyaluronic acid dissolvers as they are very effective, act quickly and have a low side-effect profile. Hope that helps. Regards, adam goodwin.

Dear miserablebird:In the early phase of hyaluronic acid (HA) filler injection, the gel will develop an encapsulation due to the healing process. It is at this time that if a firming lump develops, sterile saline or lidocaine injection can be used into the lump to break it up. Thereafter, I use various dilutions of hyaluronidase to smoothe or debulk or fully remove the HA filler placed. Hyaluronidase works quickly to break down the bonds of the filler but the residual filler will need 6 weeks to be removed by the white cells. So reschedule every 6 weeks. I hope this is helpful. The way to remove unwanted hyaluronic acid fillers like Defyne is with hyaluronidase.  Saline will not dissolve the filler.  Hyaluronidase is extremely safe.  See a doctor experienced with these treatments and you should not a problem.  

forget what you read about Hyaluronidase. It's just not true, it doesn't do bad things and is specific for HA fillers. You will waste a lot of time and money and risk making things worse as someone will talk you into putting steroids  in which will make a real proble.
