This is a difficult problem. The cartilage inside the nose is asymmetric and retracting on the left side. The best way to attempt a correction would be with surgery. Either a cartilage graft or composite graft internally can be contoured to improve the defect with a permanent result. Be sure to see a rhinoplasty specialist for consulation.

It is the safest way to control the depression in the soft tissue triangle as there are no arteries in that area that can get compromised making it safe.  Since you've already had an occlusion of your blood vessels in your nose from a previous injection, best not perform that again.  Fillers are not FDA approved for that application, and can have significant complications which you narrowly avoided. A small cartilage graft can be softened, morselized and placed in that soft-tissue triangle of the nose for a permanent effect.

it appears that you had 2 tiplastys that resulted in a uneven result. the cause was most likely improper contouring of your lower later cartilages in the first place. this resulted in a collapse of the facet area. filler will never permanently correct this, the cartilage surrounding the area of depression must be corrected and properly reconstructed.sometimes a small crushed cartilage graft placed within the facet depression would correct and residual deformity.
