Mole removal begins with a comprehensive examination and consultation to determine the likelihood of a mole being worrisome. Excisional and incisional biopsies are reserved for suspicious moles. But it’s good news that most moles are not problematic and are only cosmetic, thus leading to the option of scarless or non-surgical mole removal with lasers and RF and plasma. Excision is the really only option to get rid of the big ugly mole.  A couple of dermabrasions after the excision will help a lot to fade the mark.

The decision to remove a mole should be based on whether you would rather have a scar in its place.  Some moles exist in places where the scar can resemble a wrinkle, but other times the scar may be more obvious.  Scarring is difficult to predict.  With careful surgery, proper postoperative scar care, and a little bit of luck, some scars may result in a faint, thin, flat, light line, which, may be less bothersome to the patient than the mole.  Less aesthetically desirable scars can sometimes be managed with cortisone injections, and depending on the skin type, with laser.  Ultimately, you need to decide whether the appearance of the mole is that bothersome, and if you'd rather have a small scar in its place.A visit to a plastic surgeon's office will help you to understand if you're a candidate for the procedure, and what aesthetic outcomes are realistic in your particular case.  In order to get a qualified, ethical, and expert opinion on your surgical options and expectations, schedule an in-person office consultation with a plastic surgeon certified by the American Board of Plastic Surgery.

The only treatment that insures complete removal is to have it excised which will leave a linear scar.  The choice becomes a scar or leaving the mole as is unless it's characteristics are changing which might medically necessitate its removal.  Choose a physician who is skilled in surgical removal of facial lesions.

This is likely to be a common blue naevus but really the history is important to be sure.
If you really must remove it then it needs an incredibly skilled doctor to remove this without terrible scarring.
A shave excision is no good because the pigment will be deep. It must be removed and stitched then sent away for histology.

That mole in the middle of your upper lip is a very challenging one to remove nicely -- you have darkly pigmented skin so one tricky aspect would be that you don't want the area to appear a lot lighter than the surrounding skin once it is removed.  In other words, you don't want a light spot instead of a dark spot! I've not really seen these leave "red" spots once healed. You must be realistic that removing this mole is likely to make it better, but it might not ever be "perfect" or totally disappear.  Also, you may always face the challenge of hair growing from the area but the hairs can be plucked out if you desired. Caution is also warranted if you have ever had an keloid formation from scarring in your life.
Mole removal from the face is not an extremely involved procedure, but if you want the result to be cosmetically beautiful, wtih minimal scarring and no obvious depression or pigment change from the area where the mole was removed, you need to choose a surgeon with expertise in mole removal from the face.
