I am asking for a cartilage only rhinoplasty. Will a tip-plasty and alarplasty help me get the results I want? (Photos)

It is possible to do cartilage only surgery which includes tip plasty and alarplasty and will improve the tip significantly. The recovery is much quicker than a full rhinoplasty. RegardsDr. Janjua

photograph, you may benefit from a tip refining rhinoplasty. Your surgeon can
accomplish this by trimming, suturing, and reshaping the cartilage in your
lower nose.  Make sure you specifically look at before and after
pictures of real patients who have had this surgery performed by your surgeon
and not just a computer animation system. The most important
aspect is to find a surgeon you are comfortable with. I recommend that you seek
consultation with a qualified board-certified plastic surgeon who can evaluate
you in person.

As long as the bump is OK, then a tip rhinoplasty which involves no bones, only cartilages, can be offered to patients like you.  Without an exam, it will not be possible to fully evaluate you and give you recommendations. 

A tip only rhinoplasty can be done.  This will reduce tip bulbosity and give you a finer tip that would be more in balance with the rest of our nose.  It would be nice to reduce the hump as well but if you are fine with it, it does not need to be done.  

Many thanks for your question. To achieve a more refined nasal tip you would benefit from a cephalic trim of your alar cartilages as you've discussed and tip sutures, with or without a columella strut graft.
To bring optimal balance to your face you would ideally benefit from a dorsal reduction of the hump on your nasal bridge, with infracture to produce an improved shape of the whole nose.

Some patients are candidates for tip only procedures, others need more even if it is only the tip they notice at first. The reason is that after you change that area other areas may become more noticeable and now unsightly. You need to go for a consult with imaging. You may find that after your tip is reduced the hump seems much larger then you think it is.

It's very important that all the components of the nose balance with each other, and the entire new nose balances with the patients feel facial features. The nose is a three-dimensional structure, and a rhinoplasty is performed in three dimensions. A partial rhinoplasty will be unbalanced and unnatural in appearance, and is not recommended.

You appear to have thick skin and asymmetry as well as a dorsal hump and it looks like you would benefit from a full rhinoplasty.

I only agree to do surgery for someone if both myself and the patient agree that the game plan is acceptable. If the patient suggests something that will look worse I will not do it and by the same reasoning if I suggest something and they are not in agreement surgery shouldn't happen. Sometimes though a patient will suggest something that is reasonable and will improve their appearance but not exactly how I would approach it but in that case we can still proceed.  In your case thinning down your tip with a combination of removing some cartilage and using sutures to narrow the tip would improve the appearance of your nose and can be done as an isolated procedure but at the expense of possibly leaving the rest of your nose looking wider and bigger now by comparison.  A thorough examination and consultation with pictures showing possible outcomes in each case are necessary
