Hi there,When I smile it looks even bigger and it also drops down a bit. How much work will it require to make the look smaller? My bridge literally has a flat bone with angles on the side. I am still a nursing mother so I'll have to wait till my little one is weaned off, but want to get a rough idea. Thank you for your time!

Answer: Reduction rhinoplasty Dear Nansi, Thanks for submitting your pictures. Good communication with your surgeon is critical for good outcome, especially so with rhinoplasty surgery, since every mm counts. Verbal expression are not always clear and can lead to wrong assumptions by the surgeon and the patient. I have been in practice for 30 years and have found that life size photographs with surgical plan drawing on a tracing paper, gives the clearest idea to the patients about the upcoming changes in the nose. If the patient likes the surgical planning, than it becomes the surgery plan, however, changes in the plan can be made in the drawing according to patient's desire and surgical possibilities. From observing your pictures, the following is noticed: High bridge with small hump, wide slope, bulbous tip, wide alar base (nostrils) , over projected tip and retracted collumela. The exact changes in your nose would be based on the life size photo surgical planing, however , here is the general possible plan: 1. reduction of bridge height. 2. reduction in tip projection and refining of tip definition. 3. Narrowing nasal bony slopes and nostrils. 4. Cartilage graft to lower collumela, to increase it's show and create more feminine upturn of nose. Always, consult with experienced board certified plastic surgeons who operate in accredited surgery center for your safety. Most importantly, check the before and after pictures , to make sure that they are numerous, consistent and attractive. Best of luck, Dr Widder
