I had neck lipo performed because the doctor told me it would get rid of the sagging skin around my neck. I did't even know what the doctor was going to be doing, I just trusted him since he did a good job on my rhinoplasty. Well two years later after the surgery, I am now 20 lbs lighter, but the result is that my platysma muscles are pertruding out of my neck. I can't eat out without people staring at me. Can this be fixed? Is there some kind of procedure that can help?

Hello !

Botox can potentially reduce the visibility of platysmal bands. Botox has proven effective in treating neck bands by relaxing the neck muscles and creating a smoother appearance of the neck skin.

For more permanent and “deep” results, a platymaplasty or “neck lift” will be necessary. Platysmaplasty is a surgical approach utilized to address platysma bands. This technique entails rebuilding the platysma muscles by reattaching the separated sides of the muscles and then tightening them. Your surgeon will perform this procedure to achieve the desired outcome.

Of course, an “in person” exam is necessary. I would recommend to consult with a skilled board certified plastic surgeon for the best results.

Thank you for submitting your question and photos. Botox can reduce the appearance of neckbands, but injections are required every 3-4 months to maintain the desired results. For a more long-term solution, you may benefit from a neck lift with platysmaplasty to tighten the skin and muscles below the neck to achieve the best results.

Platysma bands are workin, functional muscle bands that become more visible as we age. It is also the case that after removing superficial fat from the neck with liposuction, muscle bands become more visible. At this stage your best optins are lower face/neck lift or extended neck lift in order to transect the muscle and performe platysmoplasty. In person consultation or a teleconference with a board certified plastic surgeo with good reputation in face and neck lifts, is recommended. Good luck.

You would require a neck lift in order to correct this. A face lift may also be required.
Liposuction can make platysmal bands more visible. Weight loss significantly increases the risk of this.

Thank you for your question. It sounds like the combination of fat removal with the liposuction and the significant weight loss has created more visibility of the platysmal bands. If the bands are only present with muscle activation, then botox can be helpful in softening their appearance. If present while you are relaxed, then some form of surgery might be needed to soften the contour. It is best to discuss this with your surgeon or any reputable Board Certified plastic surgeon in your community as there are a number of options to try and help with this.

A neck tuck under local can improve the neck bands a lot and runs roughly $8000. You can’t get rid of those pesky bands completely but u can sure hide them a lot.
