This is your anatomical feature, an obtuse cervicomental angle. Generally when you transect the platysma and pull it up to define the angle the angle is deepened and looks better.

The area of concern to you is shaped by the deep seated volume under the platysma. It is a combination of fatty tissue, the volume of the digastric muscles and the lower part of the submandibular glands. The layer of fatty tissue under the skin is relatively thin, in keeping with your low BMI.

An anterior neck lift or short scar neck lift, which is typically carried out as a deep plane neck lift technique involves reduction of a small amount of fatty tissue under the skin and the larger volume reduction is under the platysma. This can sculpt the neck and produce improved definition of the jawline. Invariably it also creates the impression that the chin is more prominent both relaxed and smiling.

The procedure does involve a scar under the chin which measures approximately 3 cm.

Based on these photos, you seem to have low positioned hyoid bone and obtuse neck. Only on the basis od in person consultation, a board certified plastic surgeon, expert in face/neck lift can make that evaluation. Neck lift is best procedure to deal with that condition. Good luck.

A mini lift with lateral platysmaplasty can correct this. Sometimes midline plication and fat removal is also needed.

Several factors help a strong neck and jawline. If there is excess fat, liposuction may help contour the neck; at times direct fat excision of the sub platysmal fat is necessary. Excess skin is addressed with a neck lift. Some people use their neck muscles significantly when laughing; botox may temporarily soften the neck platysmal muscle.

A full set of facial and neck photographs will be required to make a better determination about what is going on with your neck. From the very limited photographs, it appears that you have significant fat deposits located both above and below your platysma muscle in the neck. A surgical neck lift accomplishes removal of both compartments of fat. This consists of liposuction to remove the fat compartments above the muscle, and surgery to remove the fat deposits below the muscle which also includes a platysma plasty to significantly improve the jawline. No skin removal is required.
