Based on the pictures you’ve included, you appear to be a very good candidate for the BBL procedure. That said, it is very important to recognize that the outcome of this procedure is highly dependent on who you choose as your provider. Generally speaking, there are two variables to determine the outcome of the BBL procedure. The first one is the patient’s candidacy, and the second is provider selection. Overall these two variables are probably equally as important. Somebody who is a good candidate for the procedure has the potential of having an excellent outcome if they choose as efficiently skilled and experienced provider. They also have the ability to end up with permanent disfigurement if the procedure is not done well. Delivering consistent quality, liposuction and fat transfer results is more difficult than most people realize. The number of plastic surgeons who truly mastered these procedures are in a small minority. Recognize that the BBL is in fact, two procedures done at the same time. Patients should be assessed for both Liposuction and fat transfer just as well as patients should vet provider regarding skills for both Liposuction and fat transfer. The most common area to harvest fat from is liposuction of the torso. Personally, I view the torso as a single anatomic unit, and generally prefer to treat the entire torso when doing Liposuction. A full torso typically includes upper abdomen, lower, abdomen, love handles, waist, full back, and under arms or armpits all the way to the side of the breast. by treating the entire torso as a single anatomic unit patients are not left with transition zones between treated and untreated areas. The approach to fat grafting also varies substantially from provider to provider. I generally try to achieve a continuous, smooth S curve(a.k.a. coke bottle, hourglass) starting at the knee and finishing in the armpits. Unfortunately, some providers have a quite distorted version of what the female figures should look like and this can give the procedure a bad reputation. this includes techniques, like aggressive liposuction of the thigh in hopes that the buttocks looks larger. I called this a “lollipop look”, and I find it highly unnatural. The following are some general recommendations for finding the best provider in your community. To find the best provider, I suggest patient schedule multiple in person consultations with experienced plastic surgeons in their community. During each consultation, ask each provider to open up their portfolio and show you their entire collection of before and after pictures of previous patients who have similar body or facial characteristics to your own. An experienced plastic surgeon should have no difficulty showing you the before and after pictures of at least 50 previous patients. An experienced provider should in fact have hundreds or preferably thousands of before and after pictures to choose from for commonly performed procedures. Being shown a handful of preselected images, representing the best results of a provider’s career is insufficient to get a clear understanding of what average results will look like in the hands of each provider. There is no correct number of consultations needed to find the best provider. The more consultations you scheduled the more likely you are to find the best provider for your needs. Take careful notes during each consultation, especially regarding the quantity and quality of before and after pictures. It’s not a bad idea to bring pictures of your own body/face to use as reference when reviewing before and after pictures. The biggest mistake patients make is scheduling only one consultation without properly vetting or comparing providers before scheduling surgery. I Best, Mats Hagstrom, MD
