Will blepharoplasty change my eye shape? (Photo)

Question

considering blepharoplasty but I LOVE my eye shape and afraid it will change with surgery. I just don't want to bags! Are there non surgical options besides fillers? I notice fillers make some peoples eyes appear smaller...maybe when used in excess? just months ago no one could guess my age. Now Iook over 40 and I am 33! Any advice for my situation is appreciated!

I would suggest a trans-conjuctival blepharoplasty to remove the bags.  It will not change the shape of your eye.  This procedure simply removes the bags through an incision on the inside of the eyelid.

Given your age, a conservative approach to your eyelid concerns are indicated.  A very conservative trans-conjunctival "through-inside-of-eyelid"  approach to fat pad removal would be helpful. The skin texture can also be improved with conservative laser resurfacing.  This combination would be helpful with a low risk of affecting eyelid shape.  Best wishes.

Transconjunctival lower blepharoplasty will address the puffiness of the lower eyelids while NOT changing the eye shape. A nonsurgical treatment would be fill the area around the bags with fillers.  It may or may not be the best option for you.  See an oculoplastic surgeon.

And whatever you do, do not let anyone inject you with voluma.  This product just not seem to be ideal for the lower eyelid.  Do your homework on whomever you decide on as your surgeon.  Please proceed carefully.

If you get a lower blepharoplasty get it only Transconjunctival by a very experienced surgeon since your eyeballs stick out (prophetic) and are prone to pull down with scar. The surgery should be quite minimal or wait.

A lower lid blepharoplasty either transcutaneous or transconjunctival, to remove the lower lid fat pads, should not change the shape of your eyes.  This can happen with excess skin removal but you do not require this and so it should not affect it.  Discuss this complication with your surgeon.

Most people consider their eyelid shape to include mostly the area between their upper and lower eyelid margin, we call this the palpebral fissure. Upper and lower blepharoplasty are more designed at addressing the upper and lower eyelids above and below the lid margins. Certain techniques during blepharoplasty can be utilized to purposefully change the shape if it is needed for either functional or cosmetic reasons. For example, during lower blepharoplasty it is common to address a lax lower lid with a lateral canthopexy or canthoplasty. As I mentioned, these techniques are purposeful, planned, and are only used when needed.Your best bet is to communicate exactly what your goals are with your surgeon, that way you two can align your expectations and goals and create a treatment plan that is perfect for you, whether this includes surgery, fillers or both. Be sure to choose a Dermatologic surgeon, plastic surgeon, facial plastic surgeon, or oculoplastic surgeon who is board certified in one of these 'core four' cosmetic specialties.

Unless the image is reversed or taken in a
mirror, the left eye looks smaller than the right. It would be unwise to try to correct eye size
for purely cosmetic reasons because it will almost certainly change the eye
shape.The size discrepancy is minimal
and within the range of normal.As far as the "bags" go, your eyes have a
negative vector with less prominence of the lower orbit (bone) than the
upper.Some of it may be the angle that
you are holding your head but you already have "scleral show" (the
white visible below the iris).At your
age with nice tight canthal tendons, it's not much of an issue but, as you age,
it could cause some functional issues with tearing.Fillers could help without changing eye
shape, but are temporary and would require maintenance. Fat would be a better choice but would have
to be placed expertly and in small amounts.If fat removal is done it would also have to be done expertly and just a
small amount to avoid a skeletonized appearance.

Dear KT80,

After looking
at your pictures, I think a conservative lower blepharoplasty to remove a small
amount of fat will not change your eye shape. 
It appears that you have somewhat of an almond shaped eyes and that
should stay the same with a conservative transconjunctival lower
blepharoplasty.  The other option is to
put small amounts of filler.In your case, I would maybe use Juvederm, just
below where you have the small eyelid bags. 
A conservative amount would make your eyes look better and less puffy. I
hope that helps.
