Is avamys fluticasone furoate nasal spray safe to use if I have two small septal perforations?

I have two small septal perforations in the back of my nose due to a septorhinoplasty 7 months ago. I still have internal swelling of the turbinates and have been instructed to use a fluticasone furoate spray to reduce this swelling twice a day. Just looking for some opinions if it is safe to do so? Thanks.

Flonase or fluticasone nasal spray may improve your breathing by making your turbinates smaller. When you point the nozzle into your nose, aim for the top of your ear to avoid spraying the fluticasone directly on your septum. I doubt it would affect your small perforations, but spraying onto the septum may cause nose bleeds. Hope this helps. Dr Joseph

Thanks for your question. Nasal steroids in general are quite safe and effective for people who have nasal congestion from allergies or other reasons. One of the side effects can be dryness with bleeding. People with septal perforations are at risk of bleeding anyway (it has to do with with how airflow goes through your nose, just trust me on this one) so the nasal steroids can increase the risk when you use them. I'd probably give it a try and if you notice a problem, then stop. You could alternatively have a procedure on your turbinates to shrink them. For more, check out my blog post.
