I had a necklift, fcelift and upper and lower blepharoplasty a little over 6 months ago and I am disappointed that I still have saggy skin on my cheeks, jawline, and neck . My eyes now look more asymmetrical than before surgery. I thunk i need a revision the doctor suggests fat transfer at an additional fee. I also think my long earlobes are ugly and wonder if it is possible to shorten them.
Answer: Revision I would recommend a revisionary facelift/necklift with fat grafting, ear lobule reduction, and a deep chemical peel. I would not recommend fat transfer alone as this will not address any of the excess/loose skin. Adding volume (fat grafting) ALONE WILL NOT help to tighten the tissues/skin; it might actually create a bigger problem. This is a complex problem to address. I would recommend seeing a board-certified or board-eligible plastic surgeon with actual training in plastic surgery residency (not "facial plastics") and an aesthetic fellowship. Best of luck!

The key question to ask is whether the initial facelift effectively lifted the deeper tissues. If these were not properly addressed, it may not have achieved the desired angular jawline. Based on the appearance in these photos, a face and neck lift revision could be beneficial to address the loose skin under your neck. An earlobe reduction could also be performed simultaneously. Additionally, fat transfer to areas like the under eyes, temples, and earlobes can help create a more balanced and rejuvenated look when combined with the neck lift revision.

Answer: Facelift revision It’s hard to tell from photos how much a second facelift would help. However I can assure you that fat transfer is not the answer. Also your earlobes can be reduced quite easily

Answer: You do NOT Need Fat Transfer, Perhaps a Revision It's very hard to make judgments about your case without being the Surgeon and knowing how the procedure was performed. Some patients do have more dramatic relaxation of the their result and sometimes a more aggressive procedure is indicated. I recommend waiting another 6 months and re-assessing the results, because if a revision is indeed indicated, you want to make sure that you get the best result possible. Waiting a full year allows the chronic inflammation from your 1st procedure to subside. I would never, ever perform a fat transfer on someone with your morphology or anatomy. An earlobe reduction will address your concerns about your pendulous earlobes, which relate to pre-existing anatomy.
