28 Male, deep forehead lines, would a mixture of brow/forehead lift with a upper blepharoplasty be the best way to get rid of deep forehead lines and make eye area more youthful

A brow/forehead lift and upper bleph would be great to help with the eye area but they won't get rid of hte lines alone. You will need to continue to do botox and good skincare.

Correct, you are on spot, you have premature droopiness of the forehead-eyebrows complex, due to probable familiar inheritance, which is best treated with a short-incisions, supraperiosteal lifting of the upper two-thirds of the face; do NOT accept coronal incisions, hairline incisions or endoscopic, for a series of very very solid reasons.

Actually, you only need that forehead lift, eyelid surgery is totally unnecessary for you, this is money saving good news.

Why jump into expensive surgery that entails prolonged healing and the risks for scarring before considering a completely nonsurgical approach. Botox can be used to quiet the strong muscular tension evident in the glabella (between the eyebrows) and the forehead regions. This alone would improve the overall appearance of the forehead and the brows. However, the use of an appropriate HA filler can then treat the deeply etched -in lines that have already resulted from years of that muscular movement. If so desired, both treatments can be performed in one treatment session. Most of my patients with this issue opt for the one treatment session so that it can be "one and done." 
You would be wise to consult with an experienced, board certified cosmetic dermatologist injection expert. Hope this helps and best of luck.
