Low and large areolas pointing outwards, loose skin on cleavage making breasts fall to the side, pointy breasts.

you have a few options.  One is simply implants by themselves.  The other is to include a lift as well.  The two lifts would be either a donut type or a lollipop lift.  I like the lollipop lift. The scars heal beautifully and the areolae will not stretch out compared to the donut lift.

You
are an excellent candidate for The Bellesoma Method.  This will reshape your breast tissue creating
upper pole fullness without implants, elevate them higher on the chest wall and
more medial to increase your cleavage. 
Vertical scars are avoided, nipple sensation and the ability to breast
feed are maintained.  Your breasts will not
point outward.  At the same time or

The result you are looking for, according to the pictures you provided, will require a combination of a small breast augmentation with some type of a lift or at least an areolar reduction. Several maneuvers can be done to keep your results looking very natural: small implant, silicone, medium profile, and dual plane placement. An areolar reduction will both reduce the size of your areola and eliminate the “pointy” appearance of your breasts. Additionally, a lift can be done if you wish your nipple position to be substantially elevated.  Obviously, a consultation with an expert in more complicated breast augmentation surgery is necessary in order to formulate a specific surgical plan for you. for a breast augmentation.  The size, profile, and shape of the
implant is based on your desired breast size/shape, your chest wall
measurements, and soft tissue quality.  This decision should
be based on a detailed discussion with equal input from both you
and your surgeon.  This entire surgery
can be performed with a small incision technique.  Make sure you
specifically look at before and after pictures of real patients who have
had this surgery performed by your surgeon and evaluate their results.   The most important aspect is to find a
surgeon you are comfortable with. I recommend that you seek consultation with a
qualified board-certified plastic surgeon who can evaluate you in person.

Infinity1995.  I appreciate your concern, because size does matter when seeking a natural looking result in breast augmentation. I ask my patients to bring in photos of breasts the size they desire, because there can otherwise be confusion because B or C cup can vary greatly by brassiere manufactures! I also utilize the implant in the bra technique, with several different implant sizes with a tighter fitting T-shirt to get an idea of what you will look like. Finally, I prefer to meet with my breast implant patients at least twice before surgery, so I am confident that I know exactly what her desires regarding shape and size are. I then have several sized implants in the patient's desired range available in the OR to allow me to tailor the operation to fit her needs. Be certain your surgeon understands your desires and you should do great as this operation carries a very high satisfaction rate. Be sure to consult with a board certified plastic surgeon. Determining whether you need simply a breast implant versus a breast lift or both can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position.

Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just breast augmentation depending on the size of the implant the patient wants. I often use an implant when I perform a breast lift because it provides more upper breast fullness after the lift.

These two surgeries can be done at the same time and this may be optimal as the risks can be minimized. 

In addition, upper pole fullness is achieved with appropriate implant selection but more importantly, submuscular placement, and precise pocket dissection. The submuscular placement allows the muscle to cover the top part of the implant which allows the upper pole to appear fuller. Keeping away from the lateral chest during pocket dissection allows the implant to remain higher and prevents it from falling off to the side.

You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

Thank you for sharing your question and photographs and I think you are an excellent breast augmentation candidate to address your chief concerns.  I would stick with moderate profile implant, with high silicone fill as seen in the Natrelle Insipra lines.  Ultimately the best implant size can only be recommended after an in-person exam and measurement of key breast parameters.  Be sure to see a series of ASPS board certified plastic surgeons in consultation to discuss. 

and you really need to see a few surgeons and see what they tell you that you can do to achieve your goal photos.  Do you need a lift?  Absolutely not?  But if you want a certain look, it may help you achieve your goals but at the added costs, risks, and scarring that come with breast lifts.  Implants will provide more fullness and always can leave you with a mildly fake look and your surgeon must be absolutely careful about not making the pocket to generous laterally.  But if you just want a minor bump up in size, you could consider fat grafting, assuming you have adequate fat stores to harvest for this.  But whomever you choose, make sure they understand your goals and you need to know what happens if they don't deliver what they told you they would.  Good luck and best wishes!

You do have noticeable asymmetry in size, shape and position. You accept/like most things about your breasts but would like them a bit larger/fuller. So long as you understand and accept that implants will only make your existing breasts larger/fuller (to what ever degree you choose) without changing any other characteristics, you are a good candidate for breast augmentation. Which implant is chosen will depend on your goals and measurements. The higher the profile, the more upper pole fullness you will have so your surgeon may choose to use a lower profile implant.
