Temporal brow lift - will it give the illusion of a smaller forehead? (Photos)

A temporal lift can give a subtle “snatched” effect by lifting the outer brow, but it doesn’t actually change forehead length. Temporal lifts are best for patients who want a mild lateral brow/temple lift, but they don’t lower or shorten the forehead. If forehead length is the main issue, the only procedure that actually decreases it is a hairline-lowering (forehead reduction) surgery. An in-person exam can help determine whether brow position is truly contributing to the look or if the hairline itself is the main factor.

In order to shorten high forehead, a zig-zag hairline incision with removal of 2-3 cm of skin would be an option to consider. You do not need a brow lift since your brow shape/arch is perfect, but you need lowering of forehead by moving posterior portion of scalp into lower (distal) position and visually shortening the forehead height. The best option is a full browlift. This will raise your eyebrows and advance your hairline at the same time. For a high hairline this includes a scalp advancement which may be exactly what you want.

A temporal brow lift it’s not the answer for large forehead! The procedure that would decrease the size of your forehead while lifting your brows is the irregular Trichophytic Browlift with hairline lowering which I invented 50 years ago and have done more than 5000 of these. If properly done, you will not have a noticeable scar at the hairline since hair grows through the incision not behind it. See a very experienced surgeon for this who can show you his results with the hair, pulled back, exposing the hairline.

I will preface this by saying temporal brow lifts are one of the most common procedures I perform, and my patients love the results! BUT, a brow lift should be done to address a true problem with brow position for the best outcome, and results are more subtle than what you're showing in your photos. Your brows appear to be in great position already.
If your primary goal is to create the illusion of a smaller forehead, the best procedure for that would be a forehead reduction (also known as a hairline advancement). This can very easily be combined with a brow lift if desired at the same time. Many patients worry about the scar, but this heals very well with time.
