On several occasions, I was at the doctor (non cosmetic). The staff prepping me took a look at my paperwork, then at me, and said they cannot believe I'm actually 65+. In one case the nurse came back with another nurse and said, can you believe this patient is in her 70's? They asked for Dr. Isaacs's name. Then just last week, I was at the DMV converting my driver's license to a Real ID. The DMV clerk had the same reaction. She said, you can't be that old! You appear to have hooded upper eyelids and may benefit from upper blepharoplasty. Epicanthal folds are important to take that into consideration. The eyelid crease can also be managed if you desire. See an expert for an evaluation.
Neither canthoplasty nor canthopexy will help. I trained with Dr. Norman Shorr, the inventor of the Madame Butterfly procedure, a revolutionary approach to lower eyelid retraction. People from all over the world would fly in to have surgery. Since then we have modified this procedure to address different problems and aspects of lower eyelid retraction, since not all lower eyelid retraction is the same. We have also added newer techniques e.g. fat transfer to help with some of the morphology or anatomical variations or postoperative sequelae. See an expert for an evaluation.
As an Oculofacial plastic surgeon, a few of the procedures I perform are advanced upper and lower eyelid blepharoplasty techniques, eyelid reconstruction, revision eyelid surgery, deep-plane face and neck lifts, and revision deep-plane face and neck lifts. There are variations in surgeon’s trainings, skills, experience, and ability. The decision is up to you as the whether you want to split them up or do them together but you need to be comfortable with your decision.
I have injected many many patients with hyaluronidase, the enzyme that dissolves the filler, and it is very well tolerated. There are a lot of aesthetic/cosmetic/plastic surgeons and dermatologists that will instill fear in patients regarding hyaluronidase but again I see patients like you and I first dissolve the filler, then determine the next steps. Yes you have to be careful with any medication, but I think they exaggerate the fear factor because it garners a lot of likes and follows on social media. And the problem is that people then start looking for the wrong treatment like excising hyaluronic acid gel filler when you can simply dissolve it. Hyaluronidase will only dissolve the filler if you had hyaluronic acid gel filler by the way. If you had another type of filler, then we need to have a completely different conversation. As an Oculofacial plastic surgeon, I perform my own technique where I slightly tighten the muscle that opens the eye during the blepharoplasty. It’s ptosis surgery but a very mild version of it. This would lift your upper eyelids a bit and give you a more refreshed and awake look. You first heard it here! There are different ways to do ptosis surgery and I’m attaching a video here but this is not necessarily the way do it for you. See an expert. This has to be very carefully done. Hope his helps.
