I have malar mounds and I don't know if anything can be done to give me a nicer cheek. I am not sure if it would look natural on me because I have a lot of cheek when I smile.

This appears to be a subconjuctival hemorrhage. It looks worse than it is. It is simply a bruise on the surface of the eye. Because the "skin" of the eye is so thin, the bruise looks very red. This will go away in a week or two.
