I had a bi-lateral skin and nipple sparing mastectomy in December with immediate expander placement. I have had 3 "fills" totaling 175 cc of saline so far. My breasts were originally a bra size of 34b. After the second "fill", I noticed how extremely far apart they are, and that they seem to be coming out of my armpits. Very very unhappy so far. Is this normal placement or have they been placed too far apart?

Answer: Expander position right? Based solely on your photos, I would say that your expanders are actually pretty well centered under the nipple-areolar complexes; which is what we try to do. Perhaps they could stand to be a little closer together, but it looks as if you stated with breasts that were a bit spaced apart to begin with. I also note that the right tissue expander is a tiny bit higher than. the left. By all means share you concerns with your Plastic Surgeon, but, based on 25 years breast reconstruction experience, I will tell you than minor malpositions are extremely common. I recommend you just hang in there with the expansion. These minor issues can all be worked out at the second stage procedure when the permanent implants are put in. Best of luck.
