I had an augmentation 12yrs ago & just had the implants replaced 6m ago. Previously, my implants never felt like they shifted around much, at least not to the point of discomfort. Now, I’m constantly readjusting & I feel I need to wear a supportive bra all the time. They both, one especially, fall to the side when I lie down to the point of making almost a cavity by the breast bone. Theres a lot of rippling. It’s very uncomfortable. Is there actually something wrong with the placement?

Saline and gels are the same size if filled to the same volume. Profile can make then seem fuller. To go down one cup, I would suggest going with 300-325 cc's. A cup increase is usually 150 cc's. However, if you go too small, you may need a lift. Your surgeon can best tell you what size is best.
Why do you wish to do this to your breasts? You will thin and stretch your own tissue so much that it won't help these mammoth implants. Your neck and shoulders will have even more discomfort. Please don't do this to your breasts. They will not look good long term.
Thanks for your inquiry and pictures, without measurements it is hard to advise particulars. That being said, however all three major implant companies have extra high/full implants that have a narrower base width to projection that reach over 600 cc. Hope that helps, best of luck.
