The reason I ask is because in many plastic surgeon sites (or here) I don’t see a lot of before and after photos of fillers and usually that’s the first think I look at. What else to look for? I’ve been going to a Medi Spa for 4 years & I love my doctor, who is an aging specialist (I’m only 42), but I’m not happy with the results and want to look for a surgeon or dermatologist. You would think in FL all people care about is their bodies because is easier to find a BBL dr than a fillers one!

I agree with you in that it is not easy for the average person to find a physician/surgeon who does great work with fillers and Botox. Botox and fillers seems to be "at a glance" easy to do resulting in many physicians jumping on the band wagon to offer these services. However, Botox and dermal/soft tissue fillers is certainly not easy to do well and it takes a properly trained physician to perform these procedures safely. There are several safety concerns and potential devastating complications such as blindness, or skin necrosis that requires detailed understanding of anatomy as well as what to do in the unlikely case a complication is suspected. An artistic eye, sound aesthetic understanding and judgement along with proper technique and a "gentle hand" is paramount in providing the best outcomes. Dermal and Soft tissues fillers has evolved dramatically and it not just simply about attempting to fill a line. Its about understanding what product to use where and why... and is it going to be used very superficial, or deep dermal or it it to be used for volume restoration of the area or is it a combination there of ? Is the product best used in full strength or is it best diluted and with what and how much ? Understanding the effect of neuromodulators like Botox/Dysport on properly treated areas as well as on the adjacent areas and how to avoid complications and unwanted sequela comes from proper training and experience. Are the neuromodulators being used simply to reduce lines/wrinkles or is it being used for facial shaping or correction of asymmetry? There is alot of homework to do to find not only a properly trained physician/surgeon who performs these treatments, but one who performs these procedures safely and well. I would agree with Dr Greenberg in that a good place to start is seeking either a board certified plastic surgeon, dermatologist, facial plastic surgeon, or an ENT physician who performs these procedures on a routine basis. Understand that performing these procedure on a routine basis does not necessarily mean that they do it well either. It may be that the best physician/surgeon in performing these procedures in one convenient area or city is a plastic surgeon and in another area, it may be a dermatologist, etc. There may also be several talented choices in one area to choose from, but many more not quite as talented. It is not uncommon for some patients to be somewhat satisfied with treatments from one doctor only to find a world of difference and a much superior and dramatic outcome from another doctor more advanced in these techniques. You will need to do your research in checking their online reviews, meeting/consulting with the doctor and perhaps speaking with any previous patients if possible. Some doctors post before and after photos of these treatments which can be helpful, however many busy practices that do very fine work do not. A well educated patient will have a better chance in finding the best physician/surgeon to meet their needs and get the superior outcomes they are looking for. I hope that helps. Best wishes. With that said, since all of that is very intensive to do, doctors usually put up more "dramatic" images online to showcase the range of their practice. For dermatologists, fillers, botox, and other noninvasive treatments are the most dramatic in their practice. For facial plastic surgeons, general plastic surgeons, and oculoplastic surgeons, facelifts, breast lifts, BBL, and blepharoplasties, for example, are the more impressive images. I agree with a previous physician, that as surgeons, we know the anatomy of the face in a more intimate way than non-surgeons of the face, and we can offer patients both surgical and nonsurgical option to best treat a given issue. Oftentimes, surgeons who regularly inject fillers and wrinkle blockers are the very best as we know the anatomy intimately from years of operating on it. Surgeons, in the know, realize that the best results to rejuvenate faces involve not only surgery but medical grade skincare and re-volumizing deficient areas of the face with dermal fillers. Surgical results and even scarring can be improved with wrinkle blockers (Botox, Xeomin, Dysport) by relaxing the areas that may pull against the incisions. Find most well reviewed and respected physician injector in your area. Others may be qualified, but surgeons can be a notch better because of surgical experience and knowing the danger zones and best ways to optimize the results. I teach at national symposia on wrinkle blockers and fillers as they not only complement my surgeries but sometimes can be a replacement for lower eyelid bag surgery. Best regards.
