Periareolar incision gone wrong. What can I do to fix this? (Photo)

You have tethered scarring and a possible capsular contracture of one of your breasts, the former only possible with a 'nipple' incision, the latter much more likely with said incision. Methods used to untether the scarring include scar lysis and fat grafting, but multiple attempts may be necessary. Alternatively, you could convert to a true breast lift, but a smaller implant may be necessary to be able to do that due to lack of skin laxity.

Your scars are tethered to the capsules of your implants. You would need to have all of the scar removed and then repaired. There would be a chance that this could recur, but transposing some fat or breast tissue directly under the scar may help prevent this. Also, partial capsulectomy may help since it looks like you have some degree of contracture. Hope that helps. Best of luck!

Your periareloar scar is tethered to your implant capsule, causing the indentations. I think the best approach is to go in and remove that tethered scar tissue, and place a piece of Strattice mesh between the overlying soft tissues and the underlying capsule, to discourage reattachment.

Thank you for your question. A large portion of my practice is dedicated to revisionary breast surgery. The good news is that this is a very straight forward fix, however you do need a lollipop lift. This will achieve three things: Elevate the areolas, make the areolas smaller, and eliminate the indentation. Overall, your breasts will appear much prettier. The incisions do heal very nicely. Your nipples can be improved substantially with a revision. The are m as my techniques worth trying including converting the periareolar to a lollipop lift and reducing the diameter of the areola as well.

This can happen after the limited mastopexy techniques, and usually requires more than just revising the scar to fix. Several factors can be at play, including adherence of the skin to deeper breast tissues. Do not hesitate to get a second opinion.

Unfortunately, this is one of the risks of using a periareolar incision. It is a difficult problem to fix, but it can be done. The problem is that the scar is stuck down to the deeper tissues and the only way to correct it is to provide some soft tissue (fat or dermis) in between the surface layer of the skin and the deeper scar. I would suggest complete excision of the scar (not just at the surface, but all the way to the implant capsule) followed by careful reapproximation of the deep tissue layers. If there is no soft tissue below the scar, I would try to utilize some of the fatty tissue just below the dermis in the areola and fold it down prior to closure of the skin. I don't think that fat grafting will work.

for scars, I close all of my incisions in 5 layers and do not use absorbable sutures because they cause inflammation and worse scarring. I tape all of my incisions in the office visits once a week for six weeks after surgery to improve the appearance. After taping, I start my patients on a scar gel therapy for 3 months that they have to put on their incisions twice daily.You should discuss your concerns with your plastic surgeon as different surgeons have different approaches in scar management.

Having a revision scar in experience hands will assure you having a more favorable result. Now in your particular case I will definitely recommend having a full scar revision surgery taking the implants o out in order to brake the scaring tissue around it. It will also be advisable to get them in another anatomical plane.
