I have been researching tubular breast repair for quite a while, and want to go ahead and schedule a procedure. I have been recommended a Benelli lift procedure plus silicone implants for the best possible outcome. I have read that sometime tubular breast require multiple procedures to fix, and want to be sure I am moving forward with the appropriate first step. Any other recommendations?
Answer: Standard Treatment for Tuberous Breast Deformity Your photos show a variation of the tuberous breast deformity, in which the breast tissue develops in a more narrow, central location behind the areolae, resulting in the nipples and areolae bulging forward. Usually there is a smaller horizontal width or vertical height of the breast tissue. Breasts with this condition can be large, underdeveloped, or even have a significant difference in size and shape between the two breasts. the standard treatment in a single stage operation that includes a periareolar lift (a variation of the Benelli lift), scoring of the capsule around the breast tissue to release the constiction and allow the tissue to spread outward, and placement of a breast implant (silicone or saline) to enlarge the breasts. This usually creates a much more normal looking breast with a nice shape. Be sure to seek out a plastic surgeon who has experience treating tuberous breasts.

Answer: Would Highly Recommend Benelli Type Lift with Gland Scoring and Implant The pictures show a mild to moderate tubular deformity. There is herniation of the gland through the nipple/areolar complex. The correct procedure in my opinion is a circum-areolar mastopexy with a small to moderate sized implant and glandular scoring. I would recommend a round implant in the partial sub-muscular/dual plane position.

If it is very tight, you may benefit from an expander to stretch the skin before placing an implant. Usually however, you can undergo the placement of an implant of modest size that will stretch the skin. Eventually a large implant can be placed if that is your desire. The periareolar lift will help center the nipple/areola complex over the implant.

Answer: Benelli lift + implants for tuberous breasts Thanks for your question. The Benelli lift plus implants is a common solution to the tuberous breast shape. It is not uncommon to get a good result with one operation, but tuberous breasts can be more challenging to treat so a secondary procedure is always a possibility.

Answer: Avoid Benelli Lift, Tuberous breast deformity can be treated with subfascial placement of anatomically shaped devices. The first thing you must realize is that you do not have significant nipple ptosis. I would treat your case with a transaxillary (through the armpit) approach, aggressively stretch and score your inferior pole intraoperatively, place anatomically shaped implants that will help continue the expansion and place the implants in the "Cold-Subfascail plane) which allows more control of breast shape. You will likely need some degree of strapping and massage postoperatively for 4-6weeks. I believe this would yeild a very nice result with correction of your already widely spaced breasts. Any sub muscular or dual-plane technique will accentuate this and I would strongly steer you away from those procedures. Any further tweaks can be easily managed with fat grafting techniques but I do not think it would be necessary in your case. I hope this helps!All the best,Rian A. Maercks M.D.
