When speaking to your plastic surgeon be specific and show pictures of what you desire. These days there are various shapes and projections for the implants as well as density of the silicone gel. It would appear that you need smaller size implants to get the desired outcome but your surgeon will need to physically evaluate your breasts and perform measurements so that you can get a natural look. Make sure to pick a Board Certified Plastic Surgeon not just a Cosmetic Surgeon (These can be from varied specialties and not just plastic surgery.) I hope this has helped.

In answer to your question, you have to do a bit of both. The right breast needs to be reduced a little ( only if you want) and the left nipple needs to be lifted. There is almost no way to lower a nipple that is placed too high. You should post some before pictures so that your prior level of asymmetry can be assessed as well. Hope this helps.

Without feeling the area it appears that it is just an extra little area of fat. It may be some accessory breast tissue but does not appear to be so on the photo. It could probably be liposuctioned to flatten it out if you desire. Once again it needs to be examined by a Board Certified Plastic Surgeon in order to make an accurate diagnosis. If you are healthy you can have surgery at any age. 58 is not old and Breast Lifts and Reductions are performed quite frequently at that age. Make sure you see a surgeon who performs a lot of these procedures and who will guide you through the process. I hope this has helped.

How large you go depends on 2 things, your anatomy and your size desire.You appear to have a narrow breast so you may be limited with the size of implant that you can have placed. I think a 365 or 385 will be good for you. You will be a C cup for sure with those sizes. If you want to be larger than that then a 415 may work for you. In my practice I have patients try on the implants for a rough estimate. That usually helps with the decision making. You also need to trust your surgeon and make sure that the two of you are on the same page for sizing. I hope the has helped.

To begin with, 3D imaging is not always as accurate as stated. Secondly, you should post pictures of your natural breasts and not the enhanced image breasts to get a better opinion on what can be done.That being said, yes you can go larger than 275cc. Maybe not crazy big, but definitely fuller

From the photos that you have posted it appears that you lack some upper pole fullness and may require a slightly larger or higher profile implant but you also look like you have too much skin in the lower pole and lateral portion of your breast. I think that a revisional breast lift and implant exchange into the same sub-muscular, or dual plane pocket would be appropriate for you. You may also benefit from a biologic mesh placement for additional implant support. I do not recommend just a large implant over the muscle. Do NOT do that.

Just like anything else Plastic Surgeons have differing opinions on what procedure works best. Sometimes a tummy tuck would give the best result and sometimes the patient will only need to have liposuction. It depends on how much loose skin the patient has. I recommend that you go with the Surgeon that you feel the most comfortable with, not just the cheapest one. As in all things skill and results are what matter. I hope that this has helped.
