A low platelet count should be a concern going into any surgery but there are a number of ways to manage it. First and foremost you should investigate why your platelet count is low and make sure that there are no other medical issues to worry about. Breast augmentation is usually associated with very little bleeding but your surgeon may want you to see an internist or hematologist for some additional input and treatment planning prior to your procedure. You may also be at a higher risk for postoperative bleeding so your surgeon may want some recommendations from your other doctors on what to do in case that situation arises as well.
