I love my appearance when "cold" due my areolas being smaller/constricted and breasts slightly more rounded and would like this to be my default. I found a b&a by Dr Lauren Patrick where she corrected tuberous breasts with an areola reduction (case 2343). Could areola reduction (+ tissue expansion/correction?) achieve my desired look without implants? Also, I've seen some after AR pics where this skin is being pulled towards the scar rather than it being a flat circle, is this scarring common?
Answer: Implants You would do much better with implants. You appear too thin to provide much, if any, fat for a transfer. Plus, the implants will help maintain the shape better. I would suggest meeting with a board certified plastic surgeon and discussing your options.

You can benefit from breast implants, you appear to be too thin for a fat graft, an implant augmentation will provide the volume you desire. A board certified plastic surgeon will be able to guide you to the correct size implant for your measurements and desires. Measurements of the chest wall and breast imprint width as well as knowledge of all breast implant sizes and profiles should make this process easy. Try on a profile bra with various saline and silicone cc sizers your PS recommends under a tailored blouse, tank top, bikini, workout clothes, and other garments you typically wear so that you have the best idea of whether the garments fit as desired with the implant selected. Schedule an appointmetn with a BCPS for proper evaluation.

Tuberous breast deformity comes in all shapes and forms and at times can be treated with breast lift or areolar reduction without implants. You don't seem to have classic tuberous deformity, perhaps some mammary hypoplasia and areolar herniation on one side. In your case, I believe you will have the best result with a breast augmentation using implant. Placement of an implant in conjunction with scoring of lower pole will expand your breasts and give you a nice shape. Areolar reduction can also be done. Tuberous breasts are one of the most challenging breast shapes to correct. The implant size and shape can be determined by breast measurements during consultation. As you mentioned, you do need releasing cuts under your breast tissue to open up the breast to become less coney shape.
