I'm interested in fixing my tuberous breasts, but am not comfortable with implants, is it possible to do without?

I love my appearance when "cold" due my areolas being smaller/constricted and breasts slightly more rounded and would like this to be my default. I found a b&a by Dr Lauren Patrick where she corrected tuberous breasts with an areola reduction (case 2343). Could areola reduction (+ tissue expansion/correction?) achieve my desired look without implants? Also, I've seen some after AR pics where this skin is being pulled towards the scar rather than it being a flat circle, is this scarring common?
