I'm not sure exactly whether they're actually malar festoons or not. I noticed one day that they had just suddenly appeared. They show more prominently in different types of lighting. I'm 18 and find this a bit alarming. For treatment options I'm considering laser and would first like a face to face appointment, but for now I'm unable to get one. Will it continue to worsen? Is there any chance that it could fix itself on its own? And are there any underlying conditions that could cause this?

Answer: Under Eye Hallowness Agree that these are NOT festoons. You would be a good candidate for filler to address the volume deficiency. You would also be a good candidate for skin resurfacing to address pigmentation and texture changes. I recommend you see an eyelid or facial expert in your area to address your concerns.

Answer: Tear trough deformity in young patient This condition can be seen in such an early age in patients who have small cheek bones (malar hypoplasia). Improvement can be achieved by means of transconjuctival lower blapharoplasty, to release muscle and ad some volume by means of small fat flap underneath the groove. Correction with small implant or fat grafts may also be considered in selected patients. Fillers may also be used as a first step but, unfortunately, it will not solve the problem permanently. In person consultation with a board certified plastic or occuloplastic surgeon with good reputation in peri-orbital rejuvenation, facial implants and fat grafting is recommended. Good luck.

Answer: Orbitozygomatic groove This subtle hollowness is due to lack of volume and is very common. The technical term for this is Orbito-zygomatic groove and even young people have this anatomic feature. Fillers are ideal in this area, Restylane or Radiesse work very well with minimal swelling.
