Full tummy to vs Extended Tummy Tuck? (Photos)

Question

I'm 28yr 5'0 199lbs BMI of 39... I've been in this weight plateau for 5 yrs and it doesn't seem to change... with diet and exercise I have drop to 192 but that's my lowest... What is the difference between Extended Tummy Tuck and Full Tummy Tuck? My belly is very loose the skin is soft and stretching... What do you guys recommend? To loose 20 or 30 pounds? Or are there surgeons that do operate on patients that haved weight lost that the BMI is 39?

In this regard, I recommend you a  Tummy Tuck to get a flat abdomen and waist and back Liposculpture, getting better curves especially in the waist, improving your body contour.
Finally, I suggested take the fat we get and transfer to the buttocks and hips, to mold them and get better volume, contour and butt projection, is that we know as "Brazilian Butt Lift" (BBL).

Hello and thank you for your question. Based on your photograph, you will be a
great candidate for a tummy tuck with liposuction after some additional weight loss.  This can be done
through a low incision, all completely below the bikini line.  The belly button can be designed to look
natural and have no scars on the exterior. 
Your muscles can also be plicated resulting in a flat abdomen.  A breast lift can be performed at the same
time for improved shape and contour of your breasts. Make sure you specifically
look at before and after pictures of real patients who have had this
surgery performed by your surgeon and evaluate their results.  The most
important aspect is to find a surgeon you are comfortable with. I recommend
that you seek consultation with a qualified board-certified plastic surgeon who
can evaluate you in person.

For health and safety reasons, best to be at a BMI less than 30 or within 10 pounds of your goal weight and stable at that weight for 6 months prior to surgery for optimal results.
  
Once you have reached a healthy weight, from your photos you appear to be a good candidate for a full Tummy Tuck with liposuction to your flanks and back.
The best way to assess and give true advice would be an in-person exam.

Thank you for providing photos. An extended tummy tuck is basically a tummy tuck with a longer scar into the side.  I would say that you are a candidate for either one with liposuction to the flank and upper back area.Good luck.

I think that either would be appropriate in conjunction with liposuction of the sides and back.  BMI is but one measure of candidacy and patients must be evaluated as individuals.

Thank you for this question. The extended tummy tuck simply includes additional skin removal and a longer incision when compared to a standard tummy tuck. I suggest that with a BMI of 39 you will have a doubling of the risks for wound healing problems and infection after surgery so you should discuss these issues with your plastic surgeon.Best wishes, Dr. ALDO

You are best to lose more weight because the abdominal girth will not likely change too much with a tummy tuck if you still have a lot of visceral fat  ( fat over the organs, internally).  A full tummy tuck might be reasonable at that point.

Violet,Thanks for the questiona nd the photos.You would be best served with liposuction of your sides and back along with a full abdominoplasty (tummy tuck).  Weight loss after this procedure would be recommended as a BMI of 39 is very unhealthy and you are just below the maximum of BMI 40 allowed at my center.  You also appear to have a large amount of internal fat (called visceral fat) which is not addressed with the liposuction of tummy tuck.  Weightloss is the only cure for this fat.Best of luck with your search.Dr. T

Weight loss can be a very big challenge for many people, however, you are approaching a level of morbid obesity with all the risks to your short and long term health that go along with that.  I am not sure what you have tried for weight loss but a change in life style is needed if you wish to bring your weight down to a healthy level.  That is the best time to consider cosmetic surgery.Exercise is a stress reducer and a fitness technique and desirable to have in your life but women generally cannot control their weight with exercise.I recommend the hCG Diet to you.  One of my hCG Diet coaches has decreased her body weight by 100# and maintained it over the long term.  She did that in a series of 20# increments and then learned how to maintain it.  You could do it too! After that removal of extra skin is more likely to give you the body that you want and helps your body confidence.Other possibilities for weight loss include Weight Watchers, hypnosis, Zone Diet or seeing a nutritionist (you primary could send you).  If you continue with your current diet you will not be healthy as you go through your life and likely feel like your body is in control of you rather than you being in control of your body.Your risk of a complication including loss of life and disfigurement and deformity is very real so think it through carefully if you go forward with any surgery.
