We all want instant results, but the reality is that it may take up to 6 months for all the swelling to disappear. Things that you can do to decrease swelling, include watching your salt intake. It is not uncommon at this time in the recovery to look very different from the final result. Share your concerns with your surgeon, and keep your follow up visits.
It can take up to three months for all of the swelling both temporary and long term swelling and scar tissue to improve. At this time there is nothing else you can do except wait and address your concerns with your surgeon.
You still have a fair amount of swelling at 3 weeks which is completely normal. This swelling will often make the eyes look smaller and a little Asian at first. If you had a lower eyelid tightening this will be even more pronounced. As the swelling resolves they should look more normal.

I think though, even when healed, you are going to be some what disappointed with the results of your recent surgery. You look like you have ptosis. My best recommendation is to heal for several months and then decide if you are satisfied with the results of surgery. At that point it is much easier to know what you will need.
Looking at your photo, you still have a lot of swelling at 3 weeks and this is affecting your eyelid shape. The good news is that as the swelling resolves your eye shape should return to normal. If after several months you still have concerns you should discuss them with your surgeon.

Given that you are only 3 weeks out from surgery, you still have a lot of healing to take place. Healing times vary greatly and are largely dependent upon how much surgery was done and if you had much bleeding, which leads to more swelling and longer recovery. Some patients bleed more than others despite avoidance of known blood thinners and outstanding surgical technique. You did not provide a before picture, so there is no way to tell how your eyes differ from before. The swelling will continue to improve on its own, but I would recommend warm compresses and you could talk to your surgeon about taking NSAIDS (e.g., Alleve) and/or a short course of oral steroids. Older patients take longer to heal than younger as well. Some patients find benefit from taking arnica montana but at 3 weeks postop it is likely not to help. Stay in communication with your surgeon as they are the most informed to guide you through your healing. Best regards.
Dear Micafe, Thanks for your question and photo. You are still very early in the healing process and exhibit typical swelling and overcorrection making your lids thick, heavy and distorted. As you heal, the "Asian" look will slowly resolve. Light massage may help accelerate the process. Be sure and follow-up with your surgeon and express your concerns. Good luck.
