37 year old female trying to achieve a more youthful and smooth appearance of face especially lower face. What procedures would you recommend?

Since you are young and your jawline is not heavy yet, I would start with fillers. Properly placed fillers can really help the folds around the nose and mouth and improve hollowing in the cheeks too. Then, as you discuss things with your doctor, you can always discuss other things that may be good for you down the line. Be sure to go to an expert injector for consultation.

Although fillers could soften the nasilabial fold and the corner of the mouth somewhat , the most complete and successful approach for a smoother natural appearance would be a short scar deep plane mini facelift with emphasis on the midface.  The key factor in achieving a smoother natural appearance is to release and lift the superficial muscle layer and move together with the skin. A conventional minilift in which the superficial muscle is not release but simply dragged with sutures will not last. It is a big step but with continued refinements in technique my patients can always return to work in 10 days and often within a week when the changes of aging are early as in your case.

Morpheus 8 is a non-invasive skin and soft tissue tightening treatment that is consistently amazing in our patients. We use the Morpheus to exact fractional induced RF into the skin or tissue in order to tighten collagen and stimulate contraction of the tissue. For acne scars, it is quite useful to build collagen in patients who have atrophic scars but is not very productive for ice-pick scars which require TCA Cross. For tissue tightening, Morpheus is ideal for the face and neck and we often combine it with CO2 laser or Ultherapy to further enhance results. Chemical peels and skin resurfacing should be done at least 5 to 7 days after, but can be performed earlier, depending on treatment severity. Avoid exfoliating skin treatments before Morpheus and expect a period of 2-4 weeks for skin contraction to set in.

A mini facelift can lift the cheeks, reducing nasolabial folds and perioral lines.  Using a short scar technique can be done with scars that are hardly noticeable.

Thank you for your photos. You appear to have good mid facial width, and youthful volume retention in your face. It appears that you have the very beginning of some contour irregularities along your jawline and in your lower face. A mini facelift with plication of the SMAS layer would help straighten and smooth your jawline as well as your cheek and neck.  This can be done under local anesthesia with some sedation to help defer the cost and the recovery is quick.  Have a detailed discussion with your plastic surgeon about the best option for you.

I recommend a lip lift. A lip lift does 4 things: shortens the distance between your nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners. This takes 1 hour to perform under local anesthesia and costs $4,000. You would also benefit from a rhinoplasty to refine the tip of the nose. This would take 2 hour to perform under general anesthesia and costs $8,000. You would also benefit from a chin implant. This is done through an intra-oral approach under general anesthesia and costs $4,000.

It is very difficult to find what is happening in your neck and lower face from these two photos. Your shin appears to be smaller and pointy? Correction with chin implant, fillers or fat grafting can address this issue. However, lower face and neck may also need some correction. In person or teleconference consultation with a board certified plastic surgeon with good reputation in facial rejuvenation is recommended. Good luck.

A full set of facial photographs from all angles are going to be required to make a determination about how best to proceed. A lower face and neck lift accomplishes tightening loose facial and neck skin, tightening loose facial and neck muscles, lifting the jowls, and removing fat deposit in the neck along with a platysma plasty to significantly improve the jawline. You appear to have a recessive chin profile, and a prominent pre-jowl sulcus, so you may be a candidate for a small chin implant. Side profile photographs are necessary to make that determination.
