Do I have lower pole hypoplasia? Bilateral Constricted breast? Will a fat transfer alone make them look rounder? (photos)

Hi, thank you for sharing your question. You are a great candidate for breast augmentation. You do not have tuberous breasts. Fat transfer alone will not help you achieve your goal because the survival of the fat is unpredictable. Consult with a board certified plastic surgeon to review all your options. I hope this helps. Thank you for your question and photos.  Overall the breast has a nice shape and fullness, but the lower pole is a little “tight” or mildly “hypoplastic”.  To really round this out, an implant will be best.  For often a slightly lesser fullness or roundness, fat grafting can be a great tool, which allows you to avoid the use of an implant. 

Thank you for your question and for your photos. You have a good position of your nipples but as you correctly mentioned, there is a constricted lower pole. It is possible to perform breast surgery and abdominoplasty at the same time, but looking at the shape of your breasts, I believe that implants combined with the surgical release of the constricted lower pole during the augmentation, would be the best course of action to achieve the best breast you can have. Hello and thank you for your question. Tummy tuck and breast surgery can be performed at the same. You can have fat transfer to your breasts. There are certain factors to consider prior to undergoing fat transfer. There is a limitation in the amount fat that can be transferred successfully in one session, which is largely determined by the existing breast volume prior to the procedure. Typically, we can inject 200-250 cc per breast. Patients can expect an increase in the breast volume by 1/2 to 1 cup.  Generally speaking, donor sites are abdomen, flanks and thighs. This has the advantage of improving the contour of abdomen and waist, and simultaneous in breast volume. Another factor to consider is unpredictability of fat retention after transfer. Overall, implants are more common and have better and more reliable outcomes. My best advice is that you discuss your concerns and aesthetic goals with a board-certified plastic surgeon. I would ask to see before and after pictures of their real patients. It's important that you trust your surgeon's skills and ability to achieve the results you are looking for. Best,

Regardless what you call it (and names should be avoided to prevent a 'rubber stamp' for surgical correction), the distance from nipple to fold is short, which is a bigger, more important limiter on implant size. You will not look like your goal pic, but fat transfer as part of your tummy tuck and liposuction would be a good option. 

Your breasts are relatively low on the chest wall and asymmetrical.  One nipple is pointing more lateral than the other.  The technique I recommend is The Bellesoma Method.  This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage.  Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained.  After this, you can undergo a tummy tuck.  The tummy tuck will make your breasts look bigger by making the abdomen look smaller.  At the time of tummy tuck, fat can be transferred to the breasts if additional volume is desired.  Best Wishes,Gary Horndeski, M.D.

It seems like you can benefit from a breast augmentation with implants. Fat grafting to the breast will usually take more than one session for the desires of most patients, and fat grafting will not lift the breast to any appreciable degree. I recommend you to schedule an email, Skype or in-person consultation with a board certified plastic surgeon for proper evaluation. 

Based on both your own pictures and your ideal picture, you would get the best result from a breast augmentation using an implant.  You do not appear to have tuberous breasts.  Fat grafting to the breast can help mask little asymmetries or irregularities, but it does not add long term shape like an implant and can lead to calcifications that make mammograms more difficult.  A correctly chosen implant will help provide the desired shape.   The best thing to do is to have a few consultations with board certified plastic surgeons and see what they recommend.  Please see the below link for examples.  I hope this helps.

You do not appear to have tuberous breasts.  You are a great candidate for a breast augmentation.  The best and most reliable way to do this is with implants.  Fat grafting for primary augmentation does not have reliable long-term outcomes.   The size, profile, and shape of the implant is based on your desired breast size/shape, your chest wall measurements, and soft tissue quality.  This decision should be based on a detailed discussion with equal input from both you and your surgeon.  You are also a great candidate for a tummy tuck with liposuction.  This can be done through a low and short incision, all completely below the bikini line.  The belly button can be designed to look natural and have no scars on the exterior.   Your muscles can also be plicated resulting in a flat abdomen.  These surgeries can all be safely performed at the same time.  Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results.
