Due to fat atrophy and loss (deflation), as well as muscle and ligament insertion around lower eyelids, deformity called tear through is created. In same patient with small cheek bone this may happen very early in life. This hollowness, groove, may extend further over the cheeks due to deflation and fat loss in deep cheek fat compartments. Surgical solution should involve transconjuctival lower blepharoplasty to release muscle and ligament attachments and place fat pads and fat grafts to compensate for fat volume loss. In person consultation with a board certified plastic surgeon with good reputation in eyelid surgery and periorbital rejuvenation is recommended. Good luck.

It can be confusing to try to figure out which procedure works best for the aging eyes and who to ask. A midface lift is also a great option for many people to correct malar bags & usually leads to a more dramatic reduction of lower eyelid fat bags compared to just a blepharoplasty. The recovery & downtime is very minimal. There are usually no drains, most people can go out the same day, you can shower the next day and most patients take no pain medicine. There are never any guarantees of course with any procedure whether it is surgical or nonsurgical in nature. Surgery is not for everyone though. In today's day and age, there are many nonsurgical options to consider although surgery usually gets the most improvement specifically of the cheeks, neck and jowls. Aging liplines, volume loss in cheeks, forehead wrinkles and skin tone can be nicely improved with injectables and laser treatments.I would recommend that you see a board-certified plastic surgeon in your area that is a member of both ASPS & ASAPS and performs a lot of mini facelifts and neck lifts but also offers all the nonsurgical options such as injectables and laser treatments. From the photos it appears you have a tear trough deformity. This is can be fixed surgically by addressing the lower lid fat and cheek. Fillers could also be a good option. I recommend seeing an eyelid expert in your area.Best of luck.

Thank you for your question. As we age the fat pads in our cheeks descend, the bone around the orbit expands and the fat from around the eye bulges forward. This combination forms a hollowing under the eye called the tear trough deformity. There are many surgical and non-surgical options for correction. Dermal fillers (placed by an experienced injector) and laser resurfacing can fill the hollow and tighten the skin. Surgical options include lower lid blepharoplasty with fat removal or fat repositioning and/or skin removal. A tear trough deformity is a natural process of aging. It is a combination of tissue descent as your cheeks droop with age as well as weakening of the wall that holds back the fat around your eye. This causes the fat to bulge out and a deepened crease to form. This can be blended away with filler. For more permanent results, a lower eyelid procedure can resolve this issue. Hello, it is difficult to assess facial changes given the small area of the photo. However, from what I can see there is possible mild descent of the cheek + fat loss resulting in formation of a tear through. This can be addressed in a variety of ways including dermal filler, fat grafting, a surgical implant, or surgery to reposition the soft tissues of the midface.
