Is it possible to fix an appendectomy scar and get lower abdomen lipo at the same time? (photos)

It would be ideal to do both at the same time. You appendectomy scar is tethered to the underlying muscle, causing it to be pulled inward. Suctioning the fat of your abdomen and revising the scar will lead to an excellent result: seamless smooth contour where your scar is, and more definition of your entire abdomen.

Thank you for your question and photos. It seems that the scar tissue is stuck to the underlying muscle. You can have liposuction of the abdomen and a scar revision. It is important to excise the present scar and advance and suture the underlying fat together to prevent the skin from scarring down to the muscle again.See a board certified plastic surgeon experienced in advanced body sculpting techniques for an in person consultation/evaluation. scars can be revised and lasered. you can laser the scar and improve it slowly, or revise it surgically.
there are many options for hypertrophic scars:
1. vascular lasers (yellow laser, vbeam, excel v)
2. fractional lasers (fraxel, erbium, co2)
3. microneedling/PRP
4. silicone based scar gels with growth factors
5. excision +/- radiation

It’s important that you consult with a plastic surgeon in person so that your abdominal skin can be examined for the amount of laxity and whether the scar is tethered to the underlying fascia. it is hard to tell for sure without an examination. Based on your photos, it looks like you would benefit from a scar revision. You might not need liposuction. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

Based on your pictures, you are a good candidate for liposuction and scar correction, to improve your abdomen, back and flanks, and I think you'll get very good results. 
In general, you need to be physically healthy and at a stable weight, have realistic expectations, be a non-smoker and your BMI needs to be 30 or less. You should be back to training in 2 weeks.
I recommend you to make an appointment with a board certified plastic surgeon to talk about your goals and anything you need to know.
