My lower face seems to be sagging, with what seems to be small pockets of fat around my mouth, making me look than I am. I also have a very small chin, which I think worsens the problem. Although the pictures might give another impression, I'm normal weight. My family just seems to carry a lot of fat on their lower face... What are my options in terms of improving this area ? Looking for non- or minimally invasive procedures with little downtime. Non-permanent effects is not a problem.

Asymmetry is normal and expected. Buccal fat pad gives slimming not symmetry. Masseter botox will make you slimmer and make the jawline more loose. Fillers help give shaping and contouring. If you want perfect or close to perfect symmetry you need maxillofacial surgery or implants customized. I suggest seeing an expert to go through all options. I think the new Silhouette Instalift is a 20 minute office-based procedure that can provide some "lift" to the cheeks. This is taking it a step beyond filler without "incisional" surgery. I would also recommend Kybella to "melt" the area of fullness under the chin. Finally, adding filler to the chin can add some jawline improvement. The final thing that sometimes can help lower cheek fullness is intraoral buccal fat excision, which is removing a small bit of fat we're all born with from a small incision inside the mouth. There are a plethora of injectables that you can tap into to increase the dimensions of your chin. Kybella can be used to reduce some of the fat of the lower chin and submental area. The results you are looking for however are only going to likely be obtained from neck sculpting with liposuction and a chin implant. So you will have to balance that risk vs. reward issue that many of us face every day. Set up a consult with a board certified surgeon who will be honest and discuss the benefits and risks of each. Answer: Genetics and Small Chins Genetics certainly plays a significant role when it comes to aging of the lower face and neck. The two most important factors in determining your profile are your chin size and the position of your airway (trachea, hyoid bone, and adam's apple). Patients like yourself typically have a small-sized chin along with a forward positioned airway. This results in having minimal "structure" to hold up and support the tissue from above. Therefore, this type of anatomy often results in faster aging in the neck and jawline/jowls. Even when you were younger (ie in high school) you probably did not have a strong/dramatic profile. Although it is "minor surgery", I would recommend chin augmentation with an implant along with liposuction under the chin. In expert hands, the risks are quite minimal, and the recovery time is under one week. This combination of procedures will give you many years (even decades) of results and improvement. Depending on your region, the procedure package can cost anywhere from $4-8K. As in our Center, many times the procedures can be performed under local anesthesia with mild oral sedation. The lifetime cost of "non-invasive" treatments (fillers etc.) would far exceed the one time cost of the augmentation and liposuction.
