How can I fix open roof deformity without surgery or fillers? Is it possible to get my old nose back prior surgery? (photo)

Thank you for your question.  I'm not sure how far out you are from surgery, but often times during the first few weeks/months after surgery, the dorsum can appear wider than it will once the swelling has gone away.  If you have an open roof deformity, the only way to address this issue permanently is going to be with surgery.  You mention that you did not want osteotomies because you did not want cuts on your face, but generally osteotomies are performed without any external incisions.  Some surgeons use percutaneous osteotomies, with very small 2-3 mm incisions that usually heal without a visible scar, but this is not the most common technique.

Hello.  The only way to permanently correct an open roof deformity is with surgery.  This will likely require osteotomies to mobilize the bones and close the deformity.  Sometimes, a camouflaging graft may be needed to improve the result.  Fillers may temporarily help, but--with the most common & popular fillers today--these only last up to a year.  I would not recommend silicone, since that is permanent and fraught with high complication rates.  Once silicone is injected, it will be extremely hard to correct/remove and you have  higher risk of worsening your present condition.  The best thing is to see a board certified surgeon for good in office examination and discussion of all your options.  Take care.

Paced in the photograph it does not appear to be a true open roof deformity but just flattening.  This usually happens when an dorsal hump has been removed without lateral osteotomies that would bring together the bones and given narrow dorsum.  It can be improved without surgery by placing either as small implant or using fillers.RegardsDr. J

Osteotomies are required to close an open roof. They can be performed through an incision hidden inside the nose without any external scarring. Fillers are not a great option but could also camouflage an open roof. Unfortunately, nothing else will close or camouflage an open roof. It's difficult for me to see with 100% certainty but it does look like you have an open roof.  The most tried and true way to improve this is with osteotomies.  They can be performed internally without the evidence of external scarring.  The bones are then positioned closer together at the top.  Non surgical options won't give long lasting nor reliable results.  Consult with a board certified plastic surgeon, hopefully the one you already have experience with and ask if s/he is willing to perform osteotomies to improve the result.  Good luck to you!

Surgery is the only treatment for open roof deformity of rhinoplasty.  Osteotomies can be done without external scar of any kind.

Thanks for your question. From this picture, it's tough to say if you have a true open roof deformity or just widening of the bony dorsum. In any case, lateral osteotomies would repair the problem. These can be done through incisions inside the nose so that you wouldn't have any incisions on your face. I definitely would not recommend filler for this problem because they can impair the blood supply to your skin that already was affected by surgery. Good luck!
