Looking at the photos, the buttock-to-thigh transition appears fairly straight, with limited separation at the lower outer buttock. Liposuction can sometimes refine the upper posterior thigh, banana roll, or outer thigh area to create more definition, but it has to be done very conservatively. Removing too much fat directly under the buttock can weaken support, create a longer or more saggy-looking buttock, cause contour irregularity, or make the crease look worse. For a more rounded "P" shape, the plan often depends on whether the issue is excess fullness around the thigh/hip area or not enough buttock projection. If there is enough donor fat, selective liposuction combined with fat transfer to the buttock or hip area may be more useful than liposuction alone. An in-person exam with a board-certified plastic surgeon experienced in liposuction and BBL is needed to assess your skin quality, gluteal support, fat distribution, and whether conservative contouring, fat transfer, or no treatment in that specific area would be the safest option.
