Hello Donna.In my office you would receive a combination of fillers. I would start out with Restylane Lyft first and reverse the tendency to droop by adding volume to your cheeks. I would also consider Defyne for the deeper folds and refine for the finer folds. Next, I would use Dysport or Botox to relax the muscles pulling the corners of your mouth down. Since I do not have an entire picture of your face, it is difficult to an adequate assessment of your face. Sagging of the corners of your mouth usually involves loss of volume from the cheeks as well. I would start by replacing volume in the cheeks, using Restylane lyft which can be used deeper and gives great support. Then after that, I would use Restylane Lyft at the corners of your mouth as well. When evaluating the face, I always like to do a global assessment.
I would recommend a Silhouette Suture lift with 6 threads to suspend and lift the mid and lower face and supplementary injection with Radiesse to enhance the chin and soften the fold that disturbs you.

I agree with colleague Kalimullah that Restylane is better for this since it it sturdier than Refyne or Defyne. My suggestion is to stenghthen the skin below the mouth corners (Fern Pattern Technique) and over the cheekbone to reduce folding and sagging. In combination with volume on the cheekbones themselves.
It looks like you might be a better candidate for a facelift. Fillers may be a good adjunct, but they will likely not raise your mouth corners.

However, based on your photos, your would likely benefit from a combination approach. Often times, sagging and loose skin on the lower face and jawline are related to volume loss in the mid and upper face - I often recommend starting with fillers on the cheekbones, to help lift and contour the cheeks and jawline. I find that Voluma works particularly well in this area, and is one of the longest-lasting hyaluronic acid fillers on the market. You would also likely benefit from fillers directly in the folds below the corners of your mouth. In my experience, both Refyne and Defyne are thinner, more runny fillers, and likely would not provide enough structure to lift the corners of your mouth. I would recommend a more robust product like regular Restylane or Juvederm Ultra Plus in this area. You may also consider laser skin resurfacing treatments to help stimulate collagen and improve overall skin texture. I would recommend scheduling a consultation with a cosmetic dermatologist to discuss the best treatment options for you. Best of luck!
