Post rhinoplasty. Is the columella hanging? Does it need corrected? (Photo)

There is increased nostril show that can happen from alar retraction and /or loose hanging columella. It would be easier to make the columellar shorter than trying to fix the alar retraction. A conservative resection of the membranus septum behind the columella with reduce the nostril show. If that does not fix the problem only then think about fixing the alar retraction.RegardsDr. Janjua

It appears you do have a hanging columella. A full exam is warranted, but if this is the only isolated problem, you might be a candidate for a smaller procedure that doesn't violate the tip or bridge. Be sure your surgeon is an ABPS certified/ASAPS member (or AAFPRS equivalent) who specializes in revision rhinoplasty. Best of luck!

Yes, this is what you have. It is something your surgeon can correct with a minor surgery. Discuss this with him and if problematic seek a second opinion.

Yes there is hanging columella which is fairly common after rhinoplasty due to swelling, loss of ligamentous support and sometimes it was always present but not noticeable until after the surgery. More importantly you have alar retraction which occurs  with thin, poorly supported alar rims. This must be corrected in my opinion to give an adequate result.  Cartilage grafts will need to be placed under the edge of the nostril.  This should be done concurrent with reattaching the columella to the septum (tongue in groove). Rarely composite grafts (skin and cartilage from the ear) to accomplish this.

If you are happy with it, then it doesn't need to be fussed with. If you are unhappy with the amount of collumellar show that you have, then this can be corrected with a second operation to trim the medial crura of the lower lateral cartilage or shorten the septum. Depends on what is found when the tip is exposed. Sometimes a little of both, one, or the other.

If you are not satisfied with the appearance of the tip, you can seek to have it revised.  The columella can be shortened so as not to "show" as much from the profile view.  The shortening of the columella can exacerbate the amount of nostril show from the front view of the nose so it is a procedure which needs to take this three-dimensional aspect into consideration.  

Yes, it does look like there is too much columella show. This can often be treated with a small procedure with minimal downtime.

Thanks for your post. Your nose does not appear droopy, but you may be bothered by the appearance of the columella in profile. Columellar correction could be considered to improve the size of your nostril opening both at rest and while smiling with a minor revision. I would discuss your concerns with your surgeon and ask for recommendations regarding enhancement. Best wishes. 

Beauty is in the eyes of the beholder.    It does not matter what someone else thinks about your nose - if you like it great, if not, why not and what can be done about it.    Your tip is NOT droopy - if anything, a bit too much of the columella shows.
