Facelift surgery itself does not involve the eye regions and therefore does not impact dry eyes. With that being said, it is not uncommon for facelift patients to consider adding other simultaneous facial surgery such as blepharoplasty as additional procedures can help enhance the effect of facelift surgery. With a high-SMAS facelift technique, the surgeon is also lifting the mid-face which can help to increase support to the lower eyelid area and decrease the downward drag on the lower lid which can be a cause of drying in some patients. This type of lifting will often change the appearance and the surgical planning for the lower eyelids. Some patients can have a lot of hollowness or a “tear tough” deficiency that is often better corrected with autologous fat grafting which has an added benefit of adding additional support to the lower eyelid. There are many causes of dry eyes and many patients can still have successful facelift surgery as long as the eye dryness concerns have been a part of the surgical planning.
