Chin lipo, submentoplasty, or something else? Female, 30 yrs old, 5’7” and 155 lbs. (photos)

Question

I am female, 30 yrs old, 5’7” and 155 lbs (I’ve had this problem even at a lighter weight). I have hardly any angle between my chin and neck mchin disappear into my neck easily. I got one consult for chin lipo and the doctor didn’t seem confident he could get a good result because I don’t have much fat there. I am looking into getting a second consult. I got chin filler to see if that would help but it didn’t really affect the issue much, I think if my chin is recessed it’s only slightly

Neck liposuction will remove excess fat from the the neck area, and this can reduce the 'double chin" as well as help define the jawline. So, this may help you, BUT, it is true that there are other things going on such as some skin laxity and yes, your underlying bone structure. The excess fat is just a part of what may be bothering you. You can certainly have neck lipo, since it is easy for you to do and will definitely help things, as long as you understand the limitations and that it will not totally transform your neck appearance. There may be other options such as buccal fat removal or chin implant. You should seek a consultation with a board certified plastic surgeon.

Therefore, if chin implant + jawline implant is not performed together with submentoplasty. Unfortunately, you can't achieve the look you want. There are multiple treatment opportunities in your case. I would recommend starting with chin and jawline liposuction and a chin implant. If additional neck definition is needed, then a platysmaplasty. I don't believe a necklift is necessary.

All the photographs demonstrate a recessive Chin profile and fat deposits located both above and below the platysma muscle in your neck. Strongly consider placement of a Chin implant to augment the chin forward for better structural support for the soft tissues in the neck. A neck lift procedure accomplishes removal of both compartments of fat. The fat compartment below the platysma muscle has to be surgically extracted, while liposuction can accomplish removal of the fat deposits above the muscle. A platysma plasty is also performed to significantly improve the jawline once the sub-platysma fat has been removed.Both procedures take approximately an hour and a half under general anesthesia as an outpatient surgical procedure.

The great majority of patients who seek liposuction for the chin and neck have as their primary diagnosis facial skeletal imbalance with a mandible that is small and lacks projection. This typically presents with having an overbite dental occlusion, forming a premature double chin even if not obese, a lack of jawline definition, formation of premature jowls, and I an upper lip that has more forward projection than the lower lip. it’s possible to have a chin that projects out while the man the ball itself or the body of the mandible is undersized. The ideal treatment is a jar advancement but patients are apprehensive to have the treatment.
What creates each persons unique facial look is based on bone structure. Soft tissue coverage tends to be very consistent from person to person and what differentiates each of us into recognizable different people is based on skeletal structure. individuals who are considered attractive typically have that going for them as an ideal of bone structure and the true is also bone structure for individuals that are generally considered less attractive. Patients typically look for soft tissue solutions as due most plastic surgeons. oral surgeons often see problems based on bone structure since that is primarily the foundation of their work.
With not much fat to remove the bus section is limited. The reason for your platysma muscle draping your neck in an oblique fashion is that the origin is close to your neck because your mandible doesn’t project out as far as it would in an ideal face. There’s nothing wrong with your platysma but tightening the platysma may help lift it up a little bit to compensate.
All the soft tissue treatment options are trying to compensate for a bone based problems problem.
If you already have a projecting chin then augmenting the chin will not advance the mandible where the origin of the platysma muscle sits it only makes the chin stick out more.
Having an excessively projecting chin is not what you want because that is a problem in itself called the witches chin. most people opt not to have cranial facial rearrangement of their facial bones. by the way to make a quality assessment we need to see your entire face including your mid face and forehead. 
If you continue having consultations with plastic surgeons you’ll get the same Nondefinitive solution because there is no single simple treatment that will fix your concerns. You may want to consult with a couple of oral surgeons just to see what’s involved.
I’m sure there are plenty of plastic surgeons more talented than me and someone may have a great solution I’m not aware of but that’s what it looks like to me. anybody who tells you they can fix your problem needs to show you an impressive collection I’ll be for and after pictures before you commit to permanently changing your face.
Sometimes working with fillers can accomplish pretty miraculous transformations in the hands of the right provider. This may or may not be the case for the lower 1/3 of the face.
